Question: Are pelvic ultrasounds accurate in identifying suspiscous cysts ? What do they look fior that would make cysts more suspiscous than others? Are endomitrias a pre cancerous cyst? What happens if you do not remove endomitrias surgically.

It may be cancerous in few cases.

Ultrasound (specially trans vaginal sonography)is very sensitive in identifying pelvis cysts.It can differentiate many features of cysts like whether it is simple or complex.Simple cysts are thin walled with clear fluid,whereas complex cysts have echogenic echoes,septations etc.
If cysts have nodules,solid mass etc in it then carcinoma should be excluded.
Carcinoma should be excluded in all doubtful and risky cases.
Endometrioma can be cancerous in few cases.

Endometrioma is also a type of complex cyst and majority of complex cysts get resolve after some time.Patient only need conservative treatment.

In you case,workup for malignancy should be done and if it is negative then only follow up ultrasound is required.
