Question: Hi,

I want advices of multiple doctor.I explain my situation one year ago i have started to make a lot of tachycardia and since that time i think i suffer from cardiophobia and panic attack m i think its that anyway.I have maked multiple ecg, one echo and holter for 24h 6 months ago.I still feel tired and often pain in my chest without real reason and its really make my life a shit like im scared to go places and always control my heart beat.I have small hypertension and tachycardia than i try to stop with tachycardiam i also have really reduce my cigarette and alcohol comsuption.my question and is it possible to have a cardiac problem or situation change even i have maked all those exams 6 months ago??when i make exercice, i start recently , i also feel like i have missing beat after activity...its not easy situation to live than always be scared, also feel tired and dizzy...and my LAST question is it normal than heart beat go from 75/85 to 105/110 just from sitting pistion to stand up...im young but not make that much exercice

Detailed Answer:
Hello . I am Dr. Mody and I would be addressing your concern .
Considering you are 28 years of age and all the tests including 24 hour holter are negative makes it very very unlikely that you have any heart problem .
You have to cut down on your anxiety , because when you are anxious there is increased adrenalin in your blood stream . This will push your blood pressure and pulse up .
So when you are checking the pulse , just plain fear of ' would my pulse rate be high ?' Will increase your pulse rate from suddenly 80 to 100-110 . There should be no other reason except that for sudden change in pulse rate when you are sitting .
There is something called POTS (Postural ortho static tachycardia syn) where heart rate rises suddenly when you stand
The other possibility being atrial fibrillation, but again if it come s to normal that next very second that it's rare , atenolol will treat it and there should be some evidence in 24 holter .
However the next step would be a loop recorder/ event recorder. Here you will have have a small chip attached to the skin , which you can activate whenever you feel the pulse going high and it will recored it , also it auto recording.
Another thing is to rule out thyroid over activity.
You will have to quit smoking any how .
Nothing is wrong to you in most probability and hopefully you start living more fully and more freely .
