Posted on Mon, 12 Oct 2015
Question: Dear Dr. Kakkar,
Greeting!
In continuation with my previous premium question answered by you 10 days ago, recently at 7th week 2 days, did the HIV 1-2 with p24 scanning with lal path lab that came negative, vdrl (RPH) screening negative,HBsAG (hepatitis B) negative. Met with a skin consultant who after viewing said nothing in my penis glance skin. The only concern is about HIV which can reappear according to him after 6 months and told me to live happily.
Dear Doctor, I had fully protected exposure with condom. I have checked at 4 Th week and 7 week 2 days also with HIV 1, 2 + p24 screening. Both are negative. Can I please forget this incident and my results are conclusive or not.

Brief Answer:
Your Hiv status is conclusive and you don't need to test any further

Detailed Answer:
Hello. Thank you for writing back to me.

As far as Hiv is concerned your tests are conclusive and you don't need to worry about it. There is no need for any further Hiv testing.
Hsv i.e genital herpes can be transmitted inspite of protected sex with a condom, therefore I suggest you to take a screening test for Hsv i.e ELISA for Hsv type 1 & 2 IgG at 12-16 weeks from this sexual encounter. Not now. Genital herpes is the only thing that remains to be ruled out.
I think rest of your screening tests HBsAg for hepatitis and RPR for syphilis, are conclusive and fine.

Dear Doctor,

I am giving you a little brief about my last premium question asked to you and replied by you:

In third week of July had protected sex with CSW. It was fully protected with condom followed with a hand job. During this one month there was no signs and symptoms. During the fourth week saw a reddish shiny spot (attached with it) on the glance of the penis. In the morning it looks more red while later seems to less during day time. It is shiny,no pain. Please see attached.
Once month ago had protected sex with CSW. It was fully protected with condom followed with a hand job. During this one month there was no signs and symptoms.

Last week i.e during the fourth week, i washed my utensils and mistakenly rubbed my penis. Next morning seeing a reddish shiny spot (attached with it) on the glance of the penis. In the morning it looks more red while later seems to less during day time. It is shiny,no pain. Please see attached. Went to a STD specialist and he took sample from the spot, ordered blood and urine test for all STD. Please see the report at the end of 4th week.
All reports negative except, candida glabrata. he prescribed me itraconazole 400mg tablet in one day and itraconazolecream for seven days..

Discussed with you and as per your suggestion used triomcilone 0.1% cream. The reddish patch is no more but the place sometimes looks little shiny.

Went to test further at the end of 7th week for HIV, Hepatitis B, Herpes 1 and 2, VDRL (reports attached). Visited one dermatologist personally without any findings.

Looking for your specialized opinion as an expert in the field. Can I leave back this incident with normal sexual life with my wife. Am I conclusive or you suggest something.....

Brief Answer:
You are safe and don't need to worry about any STDs

Detailed Answer:
Hi.

You have nothing to worry. All your tests have been normal plus you did not had any skin or genital lesion which was suggestive of an STD.
Hiv was never a risk from the kind of sexual contact you had (i.e protected intercourse and followed by a handjob). Still it has been adequately ruled out by tests at 4th and 7 th week.
Screening for HBsAg, Hsv, RPR is also negative at 4 weeks and 7 weeks which I think is conclusive. (Normally Hsv seroconversion happens between 2- 12 weeks therefore ideally it should be done after 12 weeks but most of the patients have serconversion by 4-6 weeks and therefore a negative test at 7 week almost certainly rules out Hsv).
In my view you can forget about this incident and move on.
