Are Depakin, Chlordiazepoxide And Triphen The Correct Treatment For Schizophrenia?

Question: Hi
I just started seeing a client that was diagnosed with Schizophrenia. He is on Chlordiazepoxide, Depakin, Perphenazin and Triphen. I am not sure if he has the right diagnoses by his Psychiatrist. What are your thoughts? He's a middle eastern 49 years old male. I have read about clients that have been able to get off their meds just by changing their diets. Kindly give me feedback on his meds.Are these meds prescribed for Schizophrenia?

His first spiritual crash according to him was at 35 while he was hospitalized and said was turned in to a labratory mice and was terrible experience for him. I think he could have been diagnosed with PTSD as being in the War and severly traumatized for two years and not sure if they knew how to treat it. I am trying to make sure if he has the right diagnoses or he's been screwed up with the wrong meds for the past 14 years.

Yes, these medications are prescribed for schizophrenia, perphenazine particularly so. Without knowing more details about his history and symptoms as the patient tells them to me, it would be difficult for me to confirm if schizophrenia is a proper diagnosis. If he has ever had delusions or hallucinations, then schizophrenia would be a proper diagnosis. But he appears to be on a reasonable medication regimen.

Hi Doctor and Thank you.

1-Do you think he can substitute the meds he's on with injectable drugs? Would you prescribe it knowing the meds he's on if it would be possible for him to get it in Iran?

2- He told me he has episodes that he sees me outside himself and inside himself, meaning he is no longer himself and I am actually him. Should I bring to his attention that this could be visual hallucinations cause he expressed and insists on his love toward me and am trying to explain to him the Erotic Transference in Therapy and work through it with him. I need your input please? I dont want to come across as negating or rejecting his experiences.
I am being very careful, sensitive, empathic and cautious in my approach with him.

3-He also told me when I left the country, he almost died meaning he saw his soul leave his body as he was sitting in his chair and all of a sudden he saw his higher self that saved him and afterwards worked with a spiritual teacher that has been great support, teacher and friend to him. Was he dissociating or hallucinating or both?

4-Can he be off his meds just by changing his nutrition? I have heard of Schizophrenia clients getting off their meds just by changing their diets and addressing the Gut Brain issue!

Right now he's doing much better as he has started engaging in different classes with a Spiritual teacher.It is as if he has started school again but studying writing, speech and different spiritual teachers such as Eckart Tolle, Osho and Gorjeof. He's made a lot of progress and has a lot of awareness.He seems more motivated and hopeful about future. He has expressed wanting to be in a relationship with me and I have been trying to make him understand that it's not possible and ethical. He also wants me to help him pursue a career. All positive signs except am trying to direct him to engage in a healthy relationship with other women than his fantasy or wish of being with me. My intention is to help him as much as I can as he is showing a lot of interest in healing and hoping to get off his meds some day and live a normal life.

1. Not all the medications can be substituted with injectables. Only perphenazine has an injectable formulation.

2. I would indeed tell him this experience is not consistent with reality.

3. This sounds like a delusion. I would not call it a hallucination nor a dissociation.

4. No, changing nutrition will not have the same positive efficacy as medication.

I believe chronic treatment with medication will be necessary to keep him stable, as in almost all cases of schizophrenia. However, your therapy seems to be very helpful as well.

Doctor I am struggling with num 3 being a delusion, not sure why! Are you saying this is his belief and he didn't actually experience it? He reported actually seeing his spirit as light leave his body.

I finished XXXXXXX Levine's Somatic Experiencing and this client's experience would be considered as Dissociation.

I am not sure how this experience could be a delusion? I cant fit it in 3 different Types of Delusion. Help me understand please. I value your opinion. Are you saying it's Delusion of Reference, Grandeur or Paranoid?

If he physically saw light leaving his body, that would be a visual hallucination. If he believes his spirit left his body and was looking at his body, that would be a somatic delusion.
He may take it as a spiritual experience.
