Question: An anti aging clinician wants to put me on TRT, 200 mg/wk, along with Arimidex and Tamoxifen. I’m 30/198 lbs, with a history of controlled T2DM.

Are there any cardiovascular problems that could arise using TRT for 10-15 weeks? Any irreversible effects?

Hello,

In a diabetic patient, before starting testosterone therapy, a stress test is performed to rule out coronary artery disease as testosterone therapy has inotropic effects (increases the cardiac contractility, that may point out coronary artery disease).

But in your case, you are too young to have this diabetic chronic complication so testosterone therapy can be started even without performing stress test.
In obese patients, testosterone can help in losing weight and improving muscle mass, decreasing insulin resistance and improving glucose levels.
So the good effects of testosterone therapy in patients like you are beneficial comparing to side effects.

Are there any long term side effects of Arimidex/Anastrozole and Nolvadex/Tamoxifien? The anti-aging clinic wants me to "cycle off" the testosterone around 4 months from now. Will my previous levels return to normal? Will there be any lasting side effects?

Does the dose of testosterone matter? Should I start at 125 mg/week, before jumping to 200 mg/wk?

I take care of my diabetes (A1c was 5.9% last time it was checked), but I definitely have diabetes! I lift weights three times a week, run 4 to 8 miles every other day, eat whole foods with low carb and zero sugar. I take multivitamins, check my fasting glucose and blood pressure every day.

My general practitioner put me on 1000 mg metformin and 5 mg lisinopril every day, and it helps tremendously to check my blood sugar levels stable. The problem is that I cannot lose weight around my midsection. I have had my serum testosterone checked three times:

November 2014 = 334 ng/dL
September 2017 = 447 ng/dL
July 2018 = 359 ng/dL

Hi again,

Testosterone therapy, used for long term, can shrink the testicles in size since you are not utilizing them to produce testosterone.

The intensity at which the testes get suppressed varies in different men and this is dose dependent. To reduce this side effect HCG therapy is recommended.

Nonetheless, using HCG will automatically restore the testes to normal size and functionality.
From the other side, testosterone is converted to estrogens that may have symptomatic side effects (swelling/edema the most common), an antiestrogenic ( tamoxifen or Arimidex) will help to reduce this side effects.

That is why a triple therapy is often recommended.

The most common side effects of antiestrogens are- a headache, hair loss, bone pain etc.
I would suggest starting with low doses of testosterone.

Not to worry as the side effects are reversible.
