User:yes hi I am having problems understanding my mri results I have suffered chronic headaches for a long time now and have my mri results here can you help me to understand please?
Doctor:Hello Mam
Doctor:please go ahead
User:I have on my mri flair hyperintensity in the subcortical and deep white matter of the right and left frontal lobe and right parietal lobe what does that mean?
Doctor:It means that there is some myelin loss
Doctor:it could be due to normal aging process
User:I have had hypertension and headaches for awhile
Doctor:or some disorders like bipolar disorder
User:so how do they fix this I am 47
Doctor:can you tell me anything else about your memory and anxiety
Doctor:It depends on your symptoms
Doctor:if you have any neurological symptoms, then a diagnosis can be made
Doctor:MRI findings without associated symptoms mean nothing by themselves
User:my short term memory is very poor but recently my longterm is been affected I also have significant weakening on my left side
Doctor:In that case you will need a clinical exam by a neurologist to come up with a possible diagnosis
Doctor:it could be because of a small stroke that went unnoticed
User:oh ok so do I just request one from my gp?
Doctor:or it could be due to demyelinating disorders such as multiple sclerosis
User:my vision is failing as well
Doctor:You should see a neurologist as soon as possible
Doctor:Your GP should have made the arrangements as soon as he/she saw the MRI report
User:I think he was more concerned to the headaches been from a tumour or something drastic I have however not alerted him to my failing vision he knows of the weakness and wants to send me for an mri of my ls spine due to incontinence appearing as of late
User:he has me on the medication deralin 40 for my headaches and anxiety and twynsta for my hypertension
Doctor:that is understandable
Doctor:but you need to see a neurologist urgently
Doctor:I am afraid a GP will not be able to do much in your case
Doctor:and a proper diagnosis is essential
User:shall I discuss this with him and see if he feels I need one oh I was wondering would a motor vehicle accident that I had be the cause of my headaches
User:i never suffered a headache until i had a major car crash with head injuries
Doctor:You should discuss this with your GP
Doctor:car crashes do cause brain injury
User:so you can see my problem with understanding my results
Doctor:something called diffuse axonal injury
Doctor:I can understand your problems
Doctor:what I do not agree with is your withholding information from your treating doctor
User:there is however a note to the ventricles to be unremarkable for age
Doctor:whenever you consult your GP, you should tell him all your symptoms
Doctor:only then a doctor can make the right diagnosis
Doctor:ventricles need be affected
Doctor:ventricles are nothing but water filled spaces
User:well sometimes it is hard to remember what i am there for and i forget to make known
User:so don't worry about them then?
Doctor:I can most certainly understand your memory issues
Doctor:my suggestion to you is... write down your symptoms in a notebook
Doctor:whenever you experience a symptom, make a note of it.. keep the book with you all the time
Doctor:so when visit your doctor it will be much easier for you
User:i will try to remember to tell him next time what i find most frustrating is i am a qualified chef and i forget recipes and things all the time that they feel i am too much of a risk to work
Doctor:you can use your phone as well, if you have a smartphone
User:i get sore jaws a lot and sometimes numbness on my left side of face when have headache
Doctor:My dear mam, that is the reason I am requesting you to see a neurologist at the earliest
Doctor:the headaches can be a separate issue
Doctor:If I were to make any diagnosis, I will need your complete history, and would need to examine you
Doctor:only then I would look at your MRI report
Doctor:the numbness in the face could be due to headaches
Doctor:migraines can cause such symptoms
User:yes i wondered that myself ok i shall let you go i think i have a better understanding now and will see someone asap thank you for your help
Doctor:I am in no hurry
User:my headaches are daily
Doctor:please do not feel obliged
Doctor:What I would like you to do is..
Doctor:keep a notebook with you all the time
User:i wake up every morning with one it is horrible since i have been on deralin 40 however they are not quite so severe in intensity
Doctor:note down your symptoms as and when you experience them
Doctor:that is good
Doctor:when you visit your doctor, if you have all your symptoms noted down, it will help the doctor to come to a correct diagnosis
Doctor:so please do start making notes of your symptoms
User:ok i understand
Doctor:and please do consult a neurologist at the earliest
User:so things like weakness and feeling like i am falling tingling in hands etc
Doctor:is there anything else I can help you with?
User:i just thought they were from the heat
Doctor:They are all due to these hyperintense flairs on the MRI
Doctor:no they are not from heat
User:um no i think i have covered it all thanks
Doctor:Thank you very much for consulting me
Doctor:I hope you will benefit from this
User:thank you for helping me i appreciate it have a great day
Doctor:please remember to consult a neurologist
User:i shall
Doctor:You too have a great day ahead
Doctor:take care
