User :   hi i am a 28 year old male and am unable to run anymore as my left leg goes like jelly sometimes and then id end up falling, i have noticed that the left leg is significantly less in size then my right leg looks like i have lost alot of mass on that leg and im not sure what could be causing this any advise would be greatly appriciated
Doctor :   Hi
Doctor :   Hi , May I ask you a few more questions pertaining to your problem ?
User :   yes feel free to Dr
Doctor :   Since when have you been noticing this problem ?
User :   its been a few years now but iv overlooked it only recently iv noticed it seems to happen more often and iv lost nearly 35% mass on the left thigh compared to right thigh
Doctor :   Oh.
Doctor :   Do you have any pain, tingling, parasthesias ?
User :   im scared in a few more year i may not be able to walk at all
User :   yes i have alot of tingling sensations and pin and needles sometimes
Doctor :   Ok.
User :   no actual pain but does feel alot weaker
Doctor :   How about pain ?
User :   No pain to be honest just goes like jelly every now and then
Doctor :   Ok.
Doctor :   Do you feel sensatio to touch, pain, proprioception, differentiating two points on closing eyes fine ?
Doctor :   How are your distal muscle movements like foreleg, foot, knee and ankle joint ?
User :   yes i feel sensation and pain on the thigh not sure what you mean by proprioception
Doctor :   Ok,
Doctor :   Do you remember any history of spinal injury ?
Doctor :   or injury to hip, or any surgery ?
User :   Muscle movement seem fine i have noticed that i cant lift even 5kg with my left leg at the gym it is weak
User :   No history i did have a problem with my right nee about 10 years ago but that is now ok
Doctor :   Ok.
Doctor :   How about any consumption of alcohol ?
Doctor :   Or burns ?
User :   yes i do drink quite regular
User :   no burns
Doctor :   How much per week ?
User :   id say 7-8 pints
Doctor :   That is quiet a lot, dont you think.
User :   yes i am cutting down now to be honest
Doctor :   That can be one cause, alcoholic neuropathy.
User :   right
User :   just to the one leg??
Doctor :   This is a bit diffrent though , you are right as we see bilatreally.
Doctor :   I would suggest you to do a small test for Amyotrophic lateral sclerosis (ALS or Lou Gehrig's disease) or Guillain-Barre syndrome
Doctor :   Essentially this is not to be delayed : Doctor :   please folow the following steps : Doctor :   1. Immediate evaluation by a neurologist.
Doctor :   2. Detction of antibodies in spinal fluid.
Doctor :   3. Antibodies for ALS or Lou Gehrig's disease in blood.
User :   ok i went to doctors who have said to take a blood test and go to the gym more often this is why i needed a second opinion
Doctor :   Ok.
User :   ok will have to speak to the hosipital and see if they can fit me in
Doctor :   Did they discuss with you the 3 possibilities that I discussed.
User :   no they just said they want a blood test and that i need to go gym to strenthen muscles
Doctor :   What exactly did they test for ?
User :   i felt this was not the case so needed a second opinion as it is very weak for no reason
Doctor :   For Guillain-Barre syndrome.
Diagnosis is made by the clinical features, characteristic changes in the spinal fluid (from a lumbar puncture [spinal tap]), and electrical studies of the peripheral nerves and muscles, a procedure known as electromyography. The diagnosis reflects an elimination of other possibilities.
User :   iv not given the test yet its booked for 3rd of march, they ticked about 5 boxes in the sheet not actuall sure what they are
Doctor :   It would be best if you see a neurologist .
Doctor :   Ok.
User :   ok could i ask one final question on a different subject?
Doctor :   Please suggest the doctor you had seen all these probabilities .
Doctor :   And suggest him to run through these tests also .
User :   ok
User :   have we still got time to discuss one more issue?
Doctor :   3. I waould sugget on more test, that is EMG.
Doctor :   For the femoral nerev to see if it is intact.
Doctor :   Yes please go on .
User :   wen i pull my foreskin on my penis back iv recently started to get a white gooey substance appearing and its never really happened before
Doctor :   Ok,
Doctor :   Do you have any itching ?
Doctor :   Or any reddish spots ?
User :   no
User :   nope
Doctor :   Are you sexually active ?
User :   just every morning when i shower i pull it back and it is covered with a whitish gooey substance
User :   i was a few months ago no presently
Doctor :   It is usually common, as it is a remnant of semen after ejacualtion.
Doctor :   After every ejaculation or sexual intercourse , it is usually present.
User :   yeah i read that on the internet but iv not really been sexually active for a few months and still have this substance every morning
Doctor :   And it has to be cleaned, unless there is something unusual happening.
Doctor :   Do you have any nocturnal emissions ?
User :   Sorry whats that?
Doctor :   Or ejaculations after any dreams ?
User :   no
Doctor :   In bed or otherwise ?
User :   this is the case every morning
User :   no i thought that aswell so i thought id test it by doing nothing for a few weeks and still got it everyday
Doctor :   Uusally, the bulbous glands, preseminal glands secrete these every day for lubrication.
Doctor :   It is very common.
Doctor :   Do you see any infection like thing ?
Doctor :   out there ?
User :   no infection all looks normal to me
Doctor :   Any abnormal sensation , itching, inflammation ?
Doctor :   Then you need to worry.
User :   nope
Doctor :   Otherwise it is fine.
Doctor :   It is the usual physiology of human body.
User :   anyway to get rid of it as it could cause embarasment down the line
Doctor :   Like production of urine or sweat.
User :   right but its only started recently iv never had this before
Doctor :   Ok, Then the best thing would be to get a swab test,
Doctor :   A swb test would rule out if it is any fungal or bacterial or other cause ?
User :   ok
Doctor :   That is the best thing to be done to ruel out, as i cannot physiaclly examine you.
Doctor :   Otherwise a physical screening will help you .
User :   yes thats fine thank you
Doctor :   and the doctor may dioagnose it.
User :   my main concern was with the leg
Doctor :   Thank you for consulting me, same here.
User :   Thanks
Doctor :   Please get the tests done, and hope you get well soon.
Doctor :   Take care, bye for now.
