User : My friend had a fever for the past 3 days due to her wisdom tooth. She is 20 and she's been taking the antibacterial that her dentist prescribed her, but the infection seems spreading to her tonsils and tongue instead of vanishing. What should she do?
Doctor : hi
User : hi
User :Menchiari
Doctor : let me read what you have typed
User : thank you
Doctor : if the infection is not getting controlled with teh prescribed antibiotic and is spreading, she should consider meeting her dentist again to ass what could be wrong
Doctor : another possibility could be superadded infection of tonsil and tongue.
Doctor : *again to see what could be wrong
User : the problem is that she's visiting from italy and she can't meet her dentist, nor go to the hospital since she doesn't have insurance
Doctor : a swab test done from tonsil and from tongue will help in determining if it is same infection or different
User : so what if it's different?
Doctor : then in that case, she has to continue with symptomatic treatment like warm salt water gargling, brushing two times a day along with cleaning tongue
User : I see
Doctor : and take adequate pain relief with over the counter medications
User : also one other thing
User : she can barely eat or drink, since it hurts so much
User : and I know that drinking is essential
Doctor : what I meant to say is that the test suggested would tell you if it was a different infection or the same one that is affecting the tooth
Doctor : the warm salt water gargling will help in reducing the pain and with swallowing fluids and soft foods
Doctor : she should do it 2-4 times a day
User : thank you
User : I gave her hydrogen peroxide
User : does that help?
Doctor : and if still she does not improve, then she has to visit a doctor for examination and prescription of superior antibiotics to control all infections and give relief
Doctor : it can help, but it is dangerous to give it
User : why is it dangerous?
Doctor : as it can affect lungs if mistakenly enters the wind pipe
User : oh it for sure didn't affect lungs
Doctor : avoid giving it. instead you can give her mouth gargles with Listerine or betadine mouth gargles
User : sounds good
Doctor : have you followed what I have said?
User : perfectly
Doctor : is there anything else I can help you with?
User : yes
User : her tongue is white
User : and it has a white border at the sides
User : I think that's what's hurting the most
Doctor : ok, it could be due to thrush or due to antibiotic use, an examination done by a doctor would help in confirming probable cause
User : so it's a normal thing with antibiotics?
Doctor : with prolonged antibiotic use few people can have it, as the anbiotics kill the normal flora/bactweria alongw ith teh harmful ones
User : oh I see
Doctor : so in absence of normal bacteria, yeast infections can result
User : so that pain has to stay basically
User : or is there something you suggest taking
Doctor : no, the pain can be adequately controlled with painkillers or anti inflammatories.
Doctor : is she taking any of these?
User : just some acetaminophen but very very little
Doctor : ok, then you can give her Tab Tylenol 2 times a day after food. It is an OTC medicine.
Doctor : it will help in pain control and inflammation
User : thank you very much
User : you've been incredibly helpful
Doctor : is there anything else I can help you with?
User : I think this was all I needed to know so far, hopefully she'll be better in the next few days before she flies back to Italy
Doctor : thanks for consulting me. bye for now. if she is not better in 2-3 days, she should consider visiting an urgent care center.
User : thank you very much I'll keep that in mind. take care
