User :   Hi please can you tell me what may be causing my excessive daytime sleepiness
User :   First of all i should mention that I am suffering from unconjugated hyperbilirubinemia
User :   20yr old male, dont drink or smoke, not on any meds
Doctor :   May i know your weight, place of living and occupation?
User :   I get 8 hours of sleep at least but suffer from extreme daytime sleepiness

User :   weight is 10st, live in UK and I have an administrative occupation

Doctor :   have you had any other blood test?
User :   total bilirubin is 94umol, cause not yet found
User :   I have had a full blood count that just showed raised bilirubin, everything else was normal
Doctor :   any liver enzymes?
User :   LFT was also normal besides unconjugated bilirubin
User :   No all normal
Doctor :   any ultrasound done?
User :   Yes and also MRI scan both clear
Doctor :   may i know what other symptoms do you have?
Doctor :   may i know which part MRI had been done?
User :   I am suffering from daytime sleepiness, intermittent pain in upper-right quadrant and mild jaundice, and minor shortness of breath
User :   MRI of the abdomen and liver
Doctor :   i guess you might be having mild Hepatitis
Doctor :   Have you done any Hepatitis Serology tests
User :   What are those
Doctor :   Those are to check Hepatitis viral infections
User :   I'm not sure I think so, is this via a blood test?
Doctor :   Yes
User :   Then yes this has been done
User :   Blood smear is also abnormal
User :   with stomatocytes, atypical lymphs and target cells
Doctor :   In that case your symptoms match with your elevated liver bilirubin
Doctor :   I would advice you also to get tested for EBV test that is the mono spot test
User :   Could this be a liver disease?
Doctor :   Yes. EBV affects the liver and causes Hepatitis which presents late.
User :   I have had high bilirubin for 6 months now, would this virus not have already gone by now?
User :   And wouldnt it have been picked up by the tests
Doctor :   If not detected and treated it may take a while for it to clear
User :   My last LFT was at the beginning of last month, and that showed ALT - 13 and GGT - 15. Does this mean that my liver is not damaged?
Doctor :   The blood test you have done indicates a typical lymphocytes that are seen in EBV infection
Doctor :   These enzyme tests are normal
User :   So my liver is not damaged?
Doctor :   The liver damage has been contained as the liver enzymes ALT and GGT are normalized now
User :   They always have been normal they have never been high
User :   So is the most likely caused mono?
User :   And is there any treatment
Doctor :   Yes
Doctor :   Antiviral medications are available by prescriptions.
User :   Ok
User :   what are the chances this is a more serious disease eg chronic
Doctor :   Since you don't have high liver enzymes the chances are less.
User :   Ok
User :   the target cells and stomatocytes, what are these indicative of?
Doctor :   Those are indicative of rapid blood cell production which occurs in infective states.
User :   Do they point to any particular disease or condition
User :   And would you expect to see these in mono?
Doctor :   No the target cells and stomatocytes are not indicative of any specific indication.
Doctor :   Yes they are seen as a non specific finding in any viral infection
User :   Ok should i be worried?
Doctor :   No
Doctor :   You will be fine and you will recover from this condition
Doctor :   As you don't have major anatomic damage to the liver, there is nothing to fear about.

User :   with the pain over my liver it seems this is a sign of hepatitis
Doctor :   Yes it does.
User :   I am due to see the liver specialist in 10 days, should I go to the hospital or will i be ok to wait for this appointment
Doctor :   You can wait for the appointment.
User :   So most likely is mono, any other conditions that i should be aware of?
Doctor :   Hepatitis A, Hepatitis B and Hepatitis C are some of the condition you need to screened for by blood test if not already done.
