User :   HI,
I am finding small broken cashew-like particles coming out of my throat after i try to squeeze my throat to get rid of the "lump in throat" sensation. This is freaking me out. Its happened for second night now after brushing my teeth.
Doctor :   Ok
Doctor :   Let me breif you
Doctor :   It absolutely normal
Doctor :   Those white particles are nothing but dead cells
Doctor :   I also guess you recntly developed a cold
Doctor :   These are formed when there is an infection once the cells infectes are killed they pile up forming aq whitish body mostly iregular and easily crushable to paste , and also smell foul;
Doctor :   and also happens follwoing hard Gargle/ throat rinsing
User :   ya, for atleast 2 days in past week, i've woken up at 2AM because of blocked and runny nose, popped a cold pill to go back to sleep. I guess so, but i have nasal allergies too. I do occasionally get hayfever symptoms and it involves sneezing, itchy palate and so on. But it subsides too.
Doctor :   All you need to do is avoid rash gargling and use BROHEXIDINE MOUTHWASH
User :   No i don't gargle!
Doctor :   Twice a day
User :   but still am getting that "stuck" sensation in throat most of the time for like 1-2 months.
Doctor :   Did you try squeezing the throat to clear up the feeling of fullness/ discomfort
User :   its just now that i got particles out of my throat. No i didn't squeeze with hand, but only tightened throat muscles by effort and that stuck sensation is strong. As if something PHYSICALLY is there.
Doctor :   Yes it the sign of throat infection, is the particle easily crushable to paste
User :   not totally to paste, but it breaks up into smaller particles and is a little sticky though. It looks exactly like a broken cashew piece.
Doctor :   Ok thats fine if crushed hard will turn to paste,
Doctor :   All you need is an antibiotic course and throat gargle
Doctor :   And consume Hot liquids in the daY
Doctor :   If you tend to smoke plz withdraw for a few days to ease early releif
User :   Oh by the way, i am diagnosed with pulmonary tuberculosis way back in january, so after 6 months of treatment, the X-RAy didn't turn out perfectly alright, so continuing it for 3 more months, into 8th month of TB treatment.
Doctor :   Ok
User :   I take tea/coffee, but i don't take it too hot, i take warm only. i don't smoke too.
User :   im 25 yrs old and very healthy male.
Doctor :   That what i meantioned infection will cause pile up of dead white cells which is your symptom
Doctor :   *thats
User :   and from where these dead cells die? in throat ?
User :   and why?
Doctor :   Once you finish your years course you will have regression of symptoms
Doctor :   These are white blood cells who generally act against infections
Doctor :   Once they have destroyed the infective organism they will also expire and form pile ups and get excreted
Doctor :   Only when the site of infection is throat we face the cells
Doctor :   Or els ethey remain unseen as excreted with waste
User :   You mean regression of TB symptoms? Actually, my TB was diagnosed only after i had severe cough, fever, and was not able to swallow anything, i was not able to speak too for like a week. They had me under hard antibiotics for about 2 weeks, only after that suspected TB and was decided after a sputum test.
Doctor :   And for further confirmation you will have to viist an ENT specialist to visiulize exactly
User :   So this problem now is related to TB?
User :   Ya i'm planning to go. But wanted some information to pinpoint if this is related.
Doctor :   Yes , due to remenant infection
Doctor :   Just explain the tighness of throat and the particles coming out
User :   okay, TB is in chest and lungs no? how come this comes out of throat, the sensation too is mostly in very upper part of oesophagus.
Doctor :   All needed is visulaize the inner throat
Doctor :   Its facilitiates infection in throat niot TB itself its common cold
User :   i also read that some food particles gets stuck in pits around tonsils
Doctor :   * not
Doctor :   Yes but will not cause tighness of throat
Doctor :   And no discomfort at all
User :   are these exactly food particles/dead cells? or something else? No, no discomfort in swallowing or breathing. But only sensation of something stuck.
Doctor :   They often tend to get infected and release puss like material if stays for longer periods
Doctor :   Did you have fever assocoited with This problem
User :   No. No fever, no cough too (even with TB treatment going), sensation is right at the bottom of neck, around the part where the top button of shirt comes.
Doctor :   Ok
User :   and about 1 inch below it.
Doctor :   My advise is just a Antibiotic course with mouth garlgle will sort your issues
Doctor :   Its probably an throat infection
Doctor :   As a food particle i also seem to know that you brush twice a day giving it less chances for food particles to get stuck
Doctor :   And do you have very unpleasnt odour from breath
User :   But i can visit a hospital though. Got it covered under my employer. The TB got diagnosed as a "throat infection" only. so, hoping this isnt serious. Brushing is twice a day, but thinking if food can be stuck in oesophagus too? i don't have VERY unpleasant breath, but its normal breath, not like totally odourfree
Doctor :   Then mostly its an common infection and easily treatable
Doctor :   All required is confirmation of findings unless i visualize i cant confirm the treatment required
Doctor :   You will need examination from a ENT specilist to proceed further
User :   Oh sure, i'll get examined. And by the way, since the coughing has stopped, why didn't i get cleared after normal 6 months treatment?
User :   i don't cough at all.
Doctor :   Food getting struck in easophagus will cause lot of discomfort which is unbearble
Doctor :   And associated with severe nausea
User :   but there is sputum which i have to clear about once or twice a day. TB should've got cured by now.
Doctor :   TB will require a year long for complete cure
User :   and i guess this is not drug resistant TB too, since coughing totally stopped.
Doctor :   And 2 nd year medicines are given for prophylaxix
Doctor :   *prophylaxis
User :   oh, i thought it was a 6 months standard treatment
User :   what is prophylaxis? related to tB?
Doctor :   yes follwoing complete cure to we extend medications for some time to ensure complete recovery and prevent reoccurance
Doctor :   In some we also extend to 2 years
User :   DOTS treatment is advertised as 6 months. Ok, for prevention. But drug resistant TB if it was , should've been know no? I have the normal curable TB only?
User :   known*
Doctor :   The tests will reval them Regarding drug resistance
Doctor :   You probably dont seem to have drug resistant one
Doctor :   As said you responded well to intial therapy
User :   but my doctor started with 3 months of AKT-4 or 4 large pills, but after first 3 months, they shifted to AKT-2 or single pill a day.
User :   ya i responded within first month or two.
Doctor :   Generally we start with AKT4 for six months and then AKT3 or AKT2 depending on symptoms for 4 months and then shift onlt to Akt2
Doctor :   There are various regimes based on the test results for tB
Doctor :   And sputum count
User :   i don't exactly remember if it was after 3 or 4 months. i think 3. But when i went after 6 months (supposed to be stopped), X-ray still had doubt, so casually asked to continue.
User :   no sputum test whatsoever after diagnosis, since NO cough.
Doctor :   ok
Doctor :   So are continuing any Tb medicines now
User :   ya into 8th month, have to revisit probably for CT scan after finishing 9th month.
Doctor :   Yes that will help
User :   ok, time seems running out. i'll just visit ENT next?
Doctor :   And dont forget to get examinedby an ENT specialist
Doctor :   Wsihing you a speedy recovery
User :   to confirm about the throat problem?
User :   sure thank you so much
Doctor :   Thanks for consulting me
User :   god bless.
Doctor :   Bye for now
