User :   hi
Doctor :   Hi
Doctor :   How can i help you
Doctor :   Are you there?
User :   i have been ill now for 2 years with what seems to be gastrointestinal problems, i constantly feel sick, have stomach cramps in lower left of stomach, chest pains to the left of chest, constant swollen throat with big white ulcers often
Doctor :   Ok..
Doctor :   To get a better picture I need this information--Age: Gender: Occupation: Location:
User :   it all happened jus one day back in september 2006, i woke up feeling like this and at the same time had a discharge from penis, never felt right again, had numerous tests and been to my gp endless amount of times wih no success
User :   im 21
User :   male
User :   student
User :   manchester uk
Doctor :   Are you a sexually active individual?
User :   yes
Doctor :   Any STD's that you have ruled out
Doctor :   During the tests
Doctor :   Sexually transmitted diseases..
User :   been to clinic about ten times since it started with nothing but NSU. No specific infection has been detected
User :   and problem started b4 I was sexually active anyway
Doctor :   Ok..
Doctor :   Any treatment that was initiated then..
User :   been given doxycyline on numerous occasions with little success
Doctor :   Ok..
User :   had camera in stomach a year ago which said I had H pylori
Doctor :   k..
User :   2 months ago had hydrogen breath test which said I had bacterial overgrowth
Doctor :   Ok..
User :   had treatment for both with no effect
Doctor :   Are you on treatment for the same..
Doctor :   ok..
Doctor :   Then i would suspect a few conditions like..
Doctor :   Chronic gastirc ulcer.. or Chronic gas\tritis..
Doctor :   In either case you would need some tests like
Doctor :   24 hr gastric pH study..
Doctor :   to asses the acid output state.
Doctor :   Then if the Ph is more you would have to get it treated appropriately..
User :   wouldnt a gastric ulcer have shown up in the stomach camera test?
Doctor :   Yes but how recently have you got that done since if untreated it will worsen
User :   yeh was a yr ago but problem started overnite and problems av been same ever since
User :   surely a probelm like that wouldnt start overnite?
Doctor :   Initially they are very small in size and can go unnoticed to the lens and the trained gsatroentrologist too..
User :   i had been drinking heavily for the week that i developed the problem, only thing I can think of that may of caused a problem????
Doctor :   Alcohol would worsen the existing problem not create one..
User :   so these 2 problems would cause swollen throat and discharge?
Doctor :   It could also be that you have developed an infection in the gut and that has now to an Abscess
Doctor :   Nope.
Doctor :   That a pharyngitis..
Doctor :   Viral or bacterial..
Doctor :   Hence the reflected pain in your chest..
Doctor :   Due to the Abscess..
Doctor :   How ever i would like you to get the retroviral status done
Doctor :   Sinnce
Doctor :   Multiple system involvement.
User :   related?
Doctor :   Yes certainly..

User :   ok the pain in both abdominal and chest is in the left and also recently get weird slight pain in left arm and left leg???
Doctor :   pain in abdominal and chest are in left is what puts me to suspect about Abscess..
User :   how wud that cause chest pain plz?
Doctor :   and the legs and hands are
Doctor :   The dermatome rule they have the nervous supply..
Doctor :   Due to the referred pain that is around..
User :   right so its because all attched together via nerves?
Doctor :   They are the nerves that originate form the same..
User :   and this abscess u think in stomach?
Doctor :   Abscess in the Abdomen not the stomach..
User :   right so could be intestines etc?
Doctor :   Yes any where inthe gut..
User :   how do i get tested for this?
User :   a CT scan?
Doctor :   Ultrasound will be the First test.. then Hr CT

User :   u think its IBS?
Doctor :   Not for now only once i have ruled out the other conditions then as a last resort..
Doctor :   Will you need any thing else..
User :   jus one more thing, i had x rays done where i had to have this white horrible drink b4 hand, would this not show absess?
Doctor :   No that would only show any Gross morphology abnormality of the Gut or the bowel..
User :   yeh think it was for the small bowel
User :   also had camera down urethra and camera thru colon
