User :   Hi there,
I need some advice as I feel like I'm going crazy. Exactly a week ago I experienced an XXXXXXXX with no headache which was a tingling that started in my right hand and spread to both hands and arms, up my neck and face, in the backs of my calves and eventually resulted in me losing my ability to speak. The whole thing lasted for about half an hour. An hour prior to that I had a strong head pressure (no pain) hearing throbbing and pain behind the eyes, followed by zig zag lines in my sight. After a trip to ER I was given migraine meds which seemed to clear up the symptoms and felt great the next day. The problem is that it has now been a week and every day I am getting a new syptom. I have experienced a very heavy painful period, earaches and ear clicking, muscle twitching in my left index finger, tight right arm aching feeling, stiffneck, XXXXXXXX vous(twice), weakness and unsteadiness in my muscles. I have seen info on migraine postdrome but feelthis should have gone by now according to what I have read. Not all symptomsare happening every day and I have been told it is probably caused by anxiety from the initial event but am very scared.
Doctor :   Hi
User :   hi
Doctor :   Let me read the query.
Doctor :   May I know your age?
User :   I am 33 and forgot to say that this was my first migraine experience.
Doctor :   Have you lost consciousness during any of these attacks?
User :   No - no loss of consciousness and was quite aware of what was happening when I had the loss of speec
Doctor :   Ok
Doctor :   Have you had similar symptoms in the past?
Doctor :   Do you have any other medical illness such as diabetes/hypertension?
User :   No never. The only ting I can remember is my arm feeling quite numb twice prior to the recent numbness but no pins and needles and not as severe. I also had a UTI two days prior to this migraine and the migraine came on two days before my period.
Doctor :   Do you smoke/consume alcohol/use illicit drugs?
User :   No smoking, very little drinking (once a month maximum) and no drugs ever
Doctor :   Do you have any other medical illness such as diabetes/hypertension/mental illness(depression, anxiety)?
User :   Not prior to this.I went to my GP on Monday as I had a very stiff neck and was concerned and she has recommended me for counselling as she thinks I have anxiety, and I have been extremely anxious since the attack but even trying to keep calm Im getting a new think every day. Tpday I have had half hour of painful earache (still dull) which prompted me to use this service.
Doctor :   Ok
Doctor :   Do you have ear discharge/hearing disturbances?
Doctor :   Is the pain restricted to one ear?
User :   I have had hearing disturbances, they started crackling yesterday when I swallowed which I have had in the past. It then felt like they had popped and everything sounded louder than before. This was in both ears. Today I have had it again but the earache is just in the left ear today. Prior to my migraine I had hearing waves of dull and normal when I had the pressure in my head.
Doctor :   Ok
Doctor :   Let me brief you.
Doctor :   Since i am unable to perform a physical examination, I cannot pinpoint the exact cause for your symptoms, however,
Doctor :   I shall provide you with some information that should be helping you.
Doctor :   Based on the description of the symptoms your symptoms are probably due to somatic causes ( due to the psychological causes anxiety and stress).
Doctor :   The treatment in such conditions will be gradual.
Doctor :   You would need the counselling therapy, however if your symptoms are severe and is disturbing your daily activities, you may use the help of medical drugs for brief period of time to control your symptoms.
Doctor :   Since the antianxiety drugs are prescription drugs, you need to consult a psychiatrist to obtain them.
Doctor :   In the mean time, I shall also suggest you few home therapies which should also be useful.
Doctor :   1. The cracking sensation that you have while swallowing is quite normal and you need not worry about them.
Doctor :   2. you can try slow breathing exercises during these episodes.
Doctor :   With these exercises, the symptoms will reduce in a few minutes.
Doctor :   Alternatively, a cool shower can also help.
Doctor :   3. Good sleep and regular diet.
Doctor :   4. Physical exercises, meditation and yoga.
Doctor :   5. If the headaches or ear ache are severe, you can use simple analgesics such as tylenol or advil.
Doctor :   One tablet when ever you the pain is severe.
Doctor :   Have you followed what I have suggested?
User :   Yes. Thanks so much for the advice - is the earache and crackling noises attributed to anxiety also?
Doctor :   Earache and crackling can also be due to ear wax.
Doctor :   It may not be due to anxiety alone.
User :   Ok thats great. I was just concerned because it all seemed to happen at once.
Doctor :   Since you do not have any other ear symptoms, it may not be of serious concern.
User :   Thanks for your help.
Doctor :   Thanks for consulting me.
Doctor :   Bye for now.
