User :   I went to the a&e on Wednesday night with burning chest pains and sweating. Pains had been on and off for 10 day. But got worse, as I have high bp (on amlodipine/canderstan) they did ECG / X-ray and bloods - all normal, ECG 2 hours later normal. Dr said it was likely angina as family history and gave me gtn tablet, within minutes I felt a pounding head but not real relief to chest pain. Tablet took 3 hours to dissolve fully so am not sure if it worked properly. I was moved to the observation ward where another dr said he didn't think it was heart related but was acid. I have suffered mildy with heartburn after pastry etc at christmas time, also a hoarser voice and have noticed some mornings / evenings a mild burning in throat area. I was prescribed omeprazole and started it yesterday, but had a bad night of burning last night and today it doesnt really feel any bettered. Gaviscon doesn't appear to help the pain either. Does it sound like acid or my heart? Should the ppi have helped by now and is it normal to have a persistent burning in centre chest without a lot of acid reflux in the throat? Please help me.

Doctor :   Since how many days are you on PPIs ?
User :   I started 20mg of omeprazole yesterday
Doctor :   ok, well it will take minimum of 15 days for a PPi to show change
Doctor :   the dose and the drug are good
User :   Oh I didn't realise that? Can you advise why the gaviscon doesn't appear to help?
Doctor :   gaviscon acts by preventing the acid effects on the stomach mucosa , and will help in less severe cases .
Doctor :   but PPI reduces the acid out put and are good in severe cases
User :   So that's why the gaviscon doesn't really ease the symptoms. Is it normal to have the pain worse at night? What can I do to ease it. Or do I just have to wait for the ppi to work?
Doctor :   yes, its common to have pain in the nights as you fast . Take Gaviscon if you get up with pain in the night, it will help momentarily
Doctor :   but the main line of treatment is PPi
User :   Thank you I feel reassured now. I would have other symptoms if it was my heart would I?
Doctor :   yes , severe gripping pain, spreading over the entire left arm , left jaw, palpitation, sweating , breathless ness , fainting , and the episodes are very sudden and severe
User :   I do have bad pain but I would describe it as burning, some night time sweating but nothing in the day, think this is more due to being warm in bed. No breathlessness, or left jaw. Would the gripping pain feel very different
Doctor :   yes, it will almost make you do nothing , i mean half dead
Doctor :   you cant move yourself , when in a heart pain
