User :   I have crohn's . Last March I got to enjoy a perianal abcess for the first time. I needed surgery to drain it. It looks like it has come back. I went to my doctor yesterday,who put me on ciprofloxacin and cephalexin and said to come back if it did not improve in a week......some websites say antibiotics will NOT work on a perianal abcess.....which is true ? I feel more discomfort today than yesterday but I have only been on the abntibiotics for 24 hours now..
Doctor :   Most of the times the abscess are to be drained by a simple incision and drainage

and after the drainage it should be packed with a sterile gauze so that the abscess is not formed again in the drained space

If the space if left open the abscess form again
Doctor :   Having all said this, if the abscess is small enough and cannot be drained the doctors shall put the person on a course of antibiotic if they could see some improvement

User :   Yes, when I had the surgery in March , they packed the incision area , and removed the packing about 24 hours later.

User :   I am a little concerned that the abcess seems worse in the last 24 hours,how long should I give the antibiotics to work before I go to the hospital to see if it needs surgery ?

It should be the complete course the doctor prescribed you

User :   so if she gave me a 7 day course, I should at least wait 7 days,even if it continues to worsen? maybe 3 or 4 days with no improvement would be safer ?

User :   ok.....complicating things is the fact that I am scheduled to go in for an ileocecoectomym (bowel resection) on Nov. 13th , if I need surgery on the abcess , how much time must pass after the abcess surgery before the bowel surgery ?
