User :   i am 20 years old i am having severe pain in my chest, high blood pressure, shortness of breath, when a blood pressure cuff is put on my right arm my heart pain increases and my right leg also becomes in pain. I went to a doctor an ekg was done but normal when i went to er though heart when i was in pain increased to over 120 please help i am scared.

Doctor :   first let me ask u a few details about yourself

Doctor :   have you had similar episodes befpre?
Doctor :   are you currently in pain?
User :   when i was younger but nearly so severe
Doctor :   do you feel breathless even at rest? are u able to sleep?
User :   no not currently but i did yesterday and that one was so bad i could hardly move
User :   no can not sleep at all i feel like someone is crushing my chest
Doctor :   are you able to breath properly, OR do u breath in a panicky mode and breath heavily?
User :   i breath in panicky just trying to catch some sort of air
Doctor :   Do u have any fever, headache ?

User :   i do break into cold sweats
Doctor :   now, i now seem to understand that u may be having a panic attack, which can present in the similar pattern.
Doctor :   NOW take a paper bag , and breath in and out of it, when u have such kind of episodes
Doctor :   Are u able to follow me ?
User :   unfortunately that does not work i will not be nervous for any reason i am in a very relaxed state

User :   laying back reading a book talking to a friend stuff like that
Doctor :   do you have any kind of symptoms right now?
User :   yes a sharp pain in the right side of my chest
Doctor :   was any Xray done when you went to the ER?
Doctor :   On a scale of 1-10, how bad is it?
User :   yes it was i went to get an echo and the dr said i should not need one because i had one when i was twelve
Doctor :   does it affect your sleep as well?
User :   6

Doctor :   ok, can you give me your height & weight ?
User :   5 11 and my weight is 231 lbs and i have lost 40 lbs
Doctor :   does the pain increase when you lie on your right side ?
User :   no
Doctor :   does it increase when u take a deep breath , or when you bend forward?
User :   yes
Doctor :   which one, o deep inhalation or bending forwards OR both?
User :   both
Doctor :   was the ECHO done ? was it normal ?
User :   the echo was not done
User :   the doctor told me "to watch how my skinny friends eat and go off all my meds"
Doctor :   i would suggest that a Xray be done, to rule out any pneumonia

Doctor :   can you tell me which medications you are on?
Doctor :   CHEST Xray will help to rule out your condition
User :   lexapro 20 mg 1 and 1/2 tablets for depression, metformin 500 mg 1/2 twice daily for pre diabetes , ocella for polycistric ovarian syndrome, and strattera 40 mg 1 tablet for ADD
Doctor :   try taking tylenol PM to relieve your pain in the mean time.
User :   they did do the chest xray and said there was nothing there

User :   why would there be pain when i have a blood pressure cuff on my right arm in lower right leg
Doctor :   if the Xray is normal, then I am relieved, to soe\me extent.

Doctor :   can u tell me the severity of the pain that comes in your leg whrn the BP cuff is inflated?
User :   5 or 6 is the pain
Doctor :   does the pain radiate to your right leg, from your chest or your lower back?
User :   my chest
Doctor :   so the pain travels from your chest to your right leg. OK,. how long does it stay ?
User :   5 to 10 minutes if not longer
Doctor :   I feel this kind of pain is somatic and nothing to worry about.
User :   good
Doctor :   sometimes, pain can arise for unknown reasons, and can be completely benign
Doctor :   do you go to the gym ?? or are u involved in any sporting activity?
Doctor :   Being involved in sports and working out will help you symptomatically as well as it will help you sleep well at night
User :   i do not but i do work alot and do not take elevators i take stairs when i am school i used to exercise everyday but since this has started i have been so weak and tired it has been hard
User :   i could lift 3 full size filled laundry baskets up two flights of stairs i can hardly pick up one
Doctor :   sometimes pain, can demoralize you.
Doctor :   But I assure you that, since all investigations have been normal, .......
Doctor :   I expect you to be in good health.
User :   should i get an echo done on my chest or wait for a while to see if the pain subsides
User :   with more exercise and more weight loss or until my cold goes away
Doctor :   DO u have any swelling in your legs ?
User :   very little
Doctor :   do u have any cough, running nose ,nose blockage, etc
User :   cough and runny nose
User :   i have had this for a few weeks now
Doctor :   can you tell me why the ECHO was done when you were young ?
Doctor :   have you been on any antibiotics anytime?
User :   sharp pain in the chest again i had a heart murmur when i was born which was said to have closed on its own
User :   and yes amoxicillin for a week
Doctor :   I feel that these are residual symptoms of your cold, and will definitely go away in a matter of few days. i dont recommend a ECHO right now, since your chest Xray was normal.
