User :   Hi, my name is XXX I am suffering from chronic pancreatitis, recieving HHC with a PICC line recieving TPN, for the past four days experiencing extreme tenderness to RUQ....very tender to touch or move, pain is consistent and is not radiating to the the back seems localized, very concerned If it's inflammed gallbladder, liver or having acute pancreatitis, just debating whether or not I should go to the hospital to be seen. I am under the care of a GI specialist and pain management specialist at this time.
Doctor :   Hello
Doctor :   I'm Dr.XXX
Doctor :   Let me read your query.
User :   Hello Dr. XXX
User :   ok
Doctor :   I shall help you with that.
User :   Ok
Doctor :   Please give me your age,Gender, Location, Occupation, Marital status, Height and Weight
User :   47 y.o. female,XXX, XXX, XXX, 5'2" current wt @ 89 lbs
Doctor :   Ok
Doctor :   You have any other associated symptoms?
User :   fever
Doctor :   OK
Doctor :   Please rate your pain 1 to 10, 10 being maximum pain.
User :   9 pain scale
Doctor :   Ok
Doctor :   Then you need to report to hospital.
User :   sigh...ok, I was afraid of that
Doctor :   For the pain severity to be brought down,
Doctor :   Along with further investigation for possible cause of fever,
User :   ok
Doctor :   And further treatment.
User :   do you think it may be related to my pacreatitis?
User :   because it is in the proximity of the liver, pancreas and gallbladder
Doctor :   Yes, it could be acute exacerbation of your pacreatitis.
User :   Ok thank you for your help...much appreciated Dr. XXX
Doctor :   which could possibly cause you the pain.
User :   correct, that is what I am thinking
Doctor :   given the description you gave in relation to the location and radiation of pain.
User :   I am a registered nurse as well, have a little knowledge what may possibly be going on, but than I am not a MD
Doctor :   when was the last ultrasound abdomen done?
Doctor :   what did the result show?
User :   hmm...I underwent stent replacement in December for stricture in the CBC
User :   oops CBD
Doctor :   you will probably require a repeat ultrasound along with blood tests to confirm/rule out the possibility of the acute exacerbation of your chronic pancreatitis.
Doctor :   was repeat ultrasound done after the CBD stent replacement?
User :   No
Doctor :   I mean to ask a follow up ultrasound say a 1-3 months later
Doctor :   ok.
Doctor :   then a repeat ultrasound is recommended to see if there are any evidence of infection in the stent.
User :   okay, no recent Ultrasound has been done since the stents were removed
Doctor :   do mean to say that at present there is no stent inside?
User :   stents are no longer there, were removed in February
User :   correct
Doctor :   you told me you had stent replacement in December for stricture
Doctor :   so I assumed that they were there
User :   correct, my specialist only kept them in only a couple of months to prevent from embedding into the mucosal lining
User :   it was just temporary
Doctor :   also the ultrasound or a CT abdomen along with blood tests wil reveal whether your symptoms are due to inflammed gallbladder, liver or having acute exacerbation of pancreatitis
Doctor :   so I recommend you to visit the hospital and get examined and evaluated further.
User :   ok, that is what I am thinking as well, thank you for the reassurance, will go in and get seen
Doctor :   is there anything else you would like to ask me?
