User :   I have been on Triphasil for a number of years and just switched to Trinessa. I had spotting about a week before my period is supposed to start and no bowel movement for a number of days. Is this common with this switch or should i be worried about pregnancy?
Doctor :   Hello.
User :   Also, I do have MS and currently take Tysabri
User :   Hi?
Doctor :   When was your last periods?
User :   I'm supposed to start tomorrow, so my last one started around June 14
Doctor :   Ok.
Doctor :   are your periods regular?
User :   They usually are pretty exact.
Doctor :   Ok.
Doctor :   What is the color of spotting?
User :   I had to switch the date of my MS medication by a week sometime in April, but it still was pretty accurate
User :   dark red
Doctor :   Ok.
Doctor :   Since when are you suffering from constipation?
User :   I finally went a little on Friday and more today, but I think the last movement was Monday or Tuesday
User :   I just felt really stopped up most of the week. I didn't have the problem of having to force it out, but just wasn't happening
User :   I've also been eating a lot of salads lately so unsure if greens affect that or not
Doctor :   What is your age?
User :   33
Doctor :   Ok, let us discuss about your vaginal spotting first
User :   Ok
User :   What else do u need to know?
Doctor :   The medications (BCP) can cause vaginal spotting and not bleeding
Doctor :   So what I suggest you is to wait for period
Doctor :   Since the switch over has no gap ( I hope), you are still protected against pregnancy
User :   So since the spotting was dark red/ brownish I shouldn't worry too much?
User :   There was no gap. Started Trinessa on the day I would have started a new pack of Triphasil
Doctor :   YOu shouild not worry now
Doctor :   Wait for the periods till July 15th
Doctor :   If they happen, cheers
Doctor :   If not, get a Urine pregnancy test
User :   Ok. I'm pretty sure I'll see it before Thursday. Just hated that I had to deal with it 2x this month...
Doctor :   Now , coming to the constipation
User :   ok
User :   what do u need to know there?
Doctor :   Since when are you really suffering from this?
Doctor :   Is it hard / incomplete stools that you have?
User :   Not exactly sure, but probably Monday or Tuesday. Around the same time the spotting started
User :   Not really hard. Today it turned more into the opposite end of the spectrum
Doctor :   What is opposite?
User :   And I've had a little cramping, but that isn't new. Food tends to pass thru me quickly since my gallbladder removal (I'm just a medical mess, I know :))
User :   diareha
User :   not quite that extreme
User :   but fairly soft and sudeen
User :   sudden
Doctor :   So you never had the problem of constipation with your previous medication?
User :   It's happened before, but infrequently.
User :   Not a regular occurance
Doctor :   Ok, It may not be related to the switch of medications
Doctor :   I suggest you some behavioural modifications
User :   such as?
Doctor :   1) Eat very slowly, proper chewing, concentrate on the food that you eat. You may require additional 10 minutes of your time to do this. This is really worth it
User :   I normally have to eat at work so that might present a challenge. But i'll see what I can do
Doctor :   2) Consume water, 3 litres per day, one litre immediately after bed
Doctor :   3) After water, just walk for 10 minutes
User :   I try to drink a lot of water as it is
Doctor :   4) Spend some time in the loo, do not hurry up because of your burry to office'
User :   but prob not a full 3 litres
Doctor :   *hurry
User :   I'll have to see what I can do. I have a lot of interruptions at work so it's hard to take my time... All good advice though, I'll work on it! Thanks!'
Doctor :   Medications should be acting as a starting point to inculcate these behavioural changes, the medications stop, these healthy behaviours should persist for life
Doctor :   You are right, one should consider the Gall bladder removal too
Doctor :   Low fat foods help, as one function of gall bladder is fat digestion
User :   I have been drinking a lot more water (especially i the heat) but not sure how much
User :   I just don't drink much else anymore, except once in awhile
Doctor :   Then it is worth, measuring the amount
Doctor :   Cut down carbonated drinks, as they dehydrate you
User :   I don't drink many as it is
User :   And I tend to eat at least 3-4 salads a week as of late
User :   So lower fat -- viniagrette dressing, not a heavy ranch
Doctor :   Americans are recommended to eat 5 servings of vegetables and 3 servings of fruits per day
Doctor :   Imagine the amount of salads
Doctor :   be it vegetable or fruit
User :   But, I'm sure you know, most American's can't or won't follow those guidlines
Doctor :   I just want to quote before we end the session
