User :   i was due to have surgery tomoro i was having sinoplasty and fess but on saturday i developed a sore throat and earache, i went to a and e and they said it was strep throat and gave me penicillin, i have been takin that as prescribed and also paracetamol, ibuprofen adn aspirin gargle but the pain is getting worse

Doctor :   ok. please describe your symptoms in detail.
User :   red, sore throat, ear ache in one ear, no cough or pus in throat, fever, fatigue, swollen glands, difficulty swallowing
Doctor :   how many times a day are you doing the aspirin gargle?
User :   every 4 hours
User :   900mg
Doctor :   ok. alternate it with warm salt water gargling or do this warm salt water gargling 2- 4 times a day, it should help
Doctor :   continue with the rest of medications as prescribed.
Doctor :   please rate you pain intensity on scale of 1-10 with 10 being highest pain
User :   about a 7
Doctor :   ok. how many doses each of penicillin, paracetamol, ibuprofen and aspirin gargle have you taken uptil now?
Doctor :   usually the antibiotic takes effect after 3-5 days of use if infection is severe
User :   penecillin 2 pills 4xdaily since sunday morning, paracetamol same as penicilllin and ibuprofen 3x daily
Doctor :   also if the pain and symptoms have progressively worsened since the start of medications, then consider getting a culture sensitivity test done from the throat and tonsils (if tonsils are enlarged).
Doctor :   please provide age, height,w eight, details of past medical illness and treatment if any
User :   dr i saw said tonsils are not enlarged

Doctor :   is there history of diabetes, high blood pressure, cholesterol?
Doctor :   are you on any other medications?
User :   i am 36, i weigh 10 stone, i am 5 foot 6 inches, i have had full abdo hysterectomy for severe endometriosis and i have bad sinuses
User :   no diabetes or anything else

User :   no other meds

Doctor :   since the medications have not helped you in relieving the symptoms, I would recommend you to go ahead with a repeat check done with your ENT doctor for considering the swab test and culture sensitivity tests
User :   ok thank you
Doctor :   to identify the causative agent and also to see which group of antibiotics would suit the best to treat it
Doctor :   you might need higher antibiotics like broad spectrum antibiotics or the cephalosporins if the penicillin is not able to cure the infection
User :   ok
Doctor :   have you followed what I have said?
User :   today is day 3 so do you think i should give it another day or so or see ent asap?
Doctor :   is the earache better after you started with medications?
User :   no not at all
Doctor :   if the pain has been progressively getting worse, then consider seeing your ENT doc at the earliest
User :   i get relief for an hour or so after pain killers but not bein cured just masked
Doctor :   if the pain is bearable with medication use, give a try for 2 more days to see if antibiotics take their effect.
User :   ok, thank you i will try that then
Doctor :   ok, then in that case consider meeting your doctor for alternate and more potent pain killers
User :   i have other pain killers but not sure what to take
Doctor :   or try with taking Tab Tylenol 1 tab 2 -3 times a day as needed after food.
User :   i am in uk so don't think we get tylenol here
Doctor :   ok. you are already taking Ibuprofen and paracetamol
User :   yes
Doctor :   but if both are not able to give adequate pain relief then higher ones should be considered
Doctor :   like combination of analgesic and antiinflammatory
Doctor :   discuss with your ENT doctor about getting them prescribed
Doctor :   for better pain control
User :   ok i will do
