User :   Hello, I'm looking for information to help deal with my father.
About 18 years ago he fell and injured his back at work. He ocntinued to work on light duty until 2001, when he eventually could not work anymore and was forced to remain at home and collect compensation. For the two years he did not work he fell into a depressive state and began to make some very delusional statements about being watched and people wanting their money back (assume insurance or compensation board people). Then he started to say that my mother was involved with them and that she would turn on him. In 2003, a second back operation was successful and he was able to return to work with some restrictions about how much weight he could bare due to the back issues. He began taking meds for both the despression and delusions. He's currently off the depression meds and supposed to be taking the delusional meds. We're not sure if he's been taking them regularly and in the last two to three weeks he's slipped into the same state where he's highly concerned about money and being watched. He's saying that it involved even the people we would least think could ruin our family. Not sure who he's talking about, he won't say yet. He's even told my mother to watch her credit card becuase large purchases are about to surface on it. Hoping for some help.
Doctor :   Hi
Doctor :   Let me read the query.
User :   thank you
Doctor :   May I know your father's age?
Doctor :   Does he have symptoms of violent behaviour?
User :   56
User :   not violent,
User :   he attempts to hide important documents
User :   and said something about closing his bank accounts
Doctor :   Ok
Doctor :   How frequent are the delusional behaviors?
User :   they're starting to become daily
User :   he takes a med at night before bed
Doctor :   Ok
Doctor :   Apart from the delusions and hallucinations, does he have any other symptoms such as confusion, disorientation's or memory impairment?
Doctor :   Does he have any other medical illness such as diabetes/hypertension/thyroid illness?
User :   not that i'm aware of, he's just become very short in his conversation and always wanting to lay down which is where he drifts into these thoughts
Doctor :   Does he have any other medical illness such as diabetes/hypertension/thyroid illness?
Doctor :   Did he suffer a head injury during the fall?
User :   no head injury that was documented
Doctor :   Ok
Doctor :   Let me brief you?
User :   please do thanks
Doctor :   According to the symptoms that you have described, your father is probably suffering from a form of schizophrenia mostly of a paranoid variety.
User :   ok
User :   what are the treatment options?
Doctor :   As this is a chronic illness (been there for a many years) the treatment will be a gradual and slow progress.
User :   thanks for letting me know that
Doctor :   He needs treatment both in the form of medications which has to be regular and psychotherapy.
Doctor :   The type of medications , the dose and frequency of these can only be suggested after being evaluated, which would need a face to face consultation with a psychiatrist.
User :   okay, the psychotherapy is something we have not done before
User :   yes, he is taking 25 milligrams of a drug, you'll have to excuse me, i can't recall the name
User :   we were advised to give him a double dosage, any thoughts on that?
Doctor :   There are many different medications that are been used for the treatment of schizophrenia.
Doctor :   Since these are prescription medications, its not legal to prescribe those online.
User :   is there a preferred one, or does it all depend on the patient and the symptoms?
Doctor :   The most commonly used drugs belong to the category of antipsychotic medications.
Doctor :   These are the preferred medications that are used.
Doctor :   They best of these can be prescribed only after a physical examination and counselling.
User :   okay
Doctor :   Hence I suggest you to consult a psychiatrist for both medications and the course of psychotherapy.
User :   also, my father wants to stay home from work, and my mother does work, would it be unhealthy to have him home alone?
Doctor :   In severe form of schizophrenia, there have been symptoms of violent and aggressive behaviours, suicidal intentions.
Doctor :   Since your dad does not have these symptoms, I feel that he may be able to manage things alone,.
Doctor :   However, I would suggest you to first consult a psychiatrist and then take this decision after the initial assessment.
User :   to be specific about his thoughts, he thinks that the company is watching him because he needed to go on compensation and for five years the company did not grant it because his condition was not diagnosed by doctors
Doctor :   Have you followed?
User :   yes i have, thank you
User :   he also thinks that my mother might be in on it
User :   the other time around he mentioned the house being watched
User :   and lines being tapped
Doctor :   Apart from the two modalities of treatment that I have suggested, this conditions will be better solved only with the help of good family support.
User :   and he'd ask us to wisper and watch what we were saying
User :   that's what i hope we can offer
Doctor :   Hence a proper education of the family with regards to this illness is also essential, so that
User :   we are a family of four children
Doctor :   you family members may be able to provide the necessary support.
User :   how do i obtain the right educational information?
Doctor :   The psychiatrist will be addressing to these issues too.
User :   looking for the best literature to deal with the situation we've been dealt
Doctor :   Have you followed?
User :   yes, i have\
Doctor :   Do you have any other query that I can assist you with?
User :   yes, any book you would recommend i pick up?
Doctor :   There are plenty of books available on this subject.
User :   recommended one?
Doctor :   I shall provide you with the details on these over a mail to you.
Doctor :   Due to the shortage of time.
User :   thank you much, YYYY@YYYY
User :   appreciate your time
