User :   I have some point tenderness on my abdomen in the lower right quadrant. This is really the only symptom I am experiencing right now, along with alittle pain upon moving or changing position. Should I go to the ER to be checked for appendicitis or some other condition? I noticed this problem roughly 24 hours ago. Thank you.
Doctor :   Hi
User :   Hello
Doctor :   Ok.
Doctor :   Do you have any fever ?
Doctor :   Any vomitings ?
User :   No fever, or vomiting at all. A rash though.
Doctor :   Ok.
User :   I break out with rash upon any illness really.
Doctor :   Ok
Doctor :   May I know your age please ?
User :   27
Doctor :   The single most important symptom for Appendicitis is point tenderness , especially in the right quadrant.
Doctor :   RT.Lower quadrant.
User :   I see. I have that, with some rebound tenderness of increasing pain as well.
User :   Upon releasing.
Doctor :   Ok. This is again classical sign of appendicitis , rarely anything else except for some cases of peritonitis or intestinal problems .
Doctor :   Do you have any of these signs : Doctor :   Characteristic signs include 1.Rovsing's sign, 2.Psoas sign and 3.Obturator sign
Doctor :   1.Rovsing's sign is a special sign associated with appendicitis often characterized by pain in the lower region of the abdomen
Doctor :   2.Psoas sign is characterized by severe and extreme pain in the right side near the hips.
Doctor :   3.Obturator sign is often characterized by spasms in the muscles near the hips.
User :   No spasms, pain is a 2 or 3 on scale of 10. It is dull and constant. Rovsing's is most likiely positive.
Doctor :   Ok.
Doctor :   Do you have any loss of apetite ?
User :   Yes, conscious due to no wanting to exascerbate situation.
Doctor :   Ok.
User :   I still get hungry however.
User :   Outside of pain in in lower right abdomen, I feel fine.
User :   I'm just on the fence because I have no health insurance. And from what I read they will likely do an operation to be safe, do you agree?
Doctor :   I would say that if it can be localised to a single point that indicates that the appandicitis has reached peritoneal level and is showing up as point tenderness, which is called Mc.Burney's point.
Doctor :   Yes you are absolutely right, you can be put on antibiotic treatment , but there is a chnace of recurrence, hence surgery is best advised in your case, and you need to go to ER.
User :   I had a severely sore throat over the past 4-5 days, do you think it could be systemic or somehow related?
Doctor :   Please get a Ultrasound done, that will make the diagnosis.
User :   Ok, thank you doctor.
Doctor :   Ok,But usually cough is very much not related to Appendicitis.
Doctor :   It cannot go systemically to appendix .
Doctor :   They might be different entities.
User :   Do you think I have just caught this very early and have not developed other symptoms yet? Do some not experience these symptoms at all?
Doctor :   Some of them dont experience any symptoms except for a mild discomfort.
Doctor :   Some times you see vague symptoms like even fever
User :   I took augmentin 875/125 for two days and the sore throat went away.
Doctor :   Ok.
Doctor :   Do you have a history of gastritis ?
Doctor :   Or ulcer ?
User :   Did this affect the flora in my gut adversly and possibly caused this?
Doctor :   or acidity ?
User :   I have had gastritis.
User :   very little acidity
Doctor :   Yes that is what i am suspecting .
Doctor :   Do you have any belching ?
User :   I immediately stopped taking the augmentin upon developing this abdominal pain.
User :   no belching.
Doctor :   Ok.
User :   but there is a perceivable backing up in the stomach when ingesting anything.
Doctor :   I would suggest one step to differentiate both .
User :   it feels backed up.
Doctor :   Ok.
Doctor :   Do you have any variation in pain after you take food or before you take food ?
User :   No, after 20-30 minutes I can feel motility through the stomach though.
User :   No pain. abdominal pain is very dull and constant.
User :   possibly a 1-2 on scale of 10.
Doctor :   oK.
Doctor :   I suggest you to take a 12 hr dose of antacids , if it subsides , you can take it for granted that it might be due to gastritis and antibiotics
Doctor :   If not you have to hit an ER.
User :   Thank you so much doctor, I will. I suspected the Augmentin because I took it on an empty stomach after rigorous workout and heavy drinking from 24 hours before.
Doctor :   I suggest you to take Alu-Cap thrice daily 3 sppons
User :   Ok, thank you so much Doctor. I appreciate it.
Doctor :   at 4 hr interval.
Doctor :   for next 12 hrs.
Doctor :   it is an OTC.
Doctor :   tHANK YOU.
