User :   Every so often I get bleeding and sometimes pain from tears or cuts in my foreskin when fully retracted. It's as if the skin is less elastic than it once was. Can you advise please?
Doctor :   Hi,
User :   Hello
Doctor :   May i know since when do you have this problem ?
User :   It's just happened again in the last couple of days _ I noticed bleeding when I went pee, but it has happened before, sometimes during sex
User :   It seems to affect the same area each time
Doctor :   ok, Do you bleed in the urine in the start or at the end of urination ?
User :   no, it's not in the urine, it's definitely from the foreskin
User :   my foreskin was retracted slightly so I noticed
User :   in fact it was fully retracted as that's when it seems to stretch and casuse the tear
Doctor :   ok, do the cuts develop into ulcers later on ?
User :   normally they heal
User :   in a few days or so
User :   as long as I abstain from sex etc
Doctor :   ok, do you use lubricants during the intercourse ?
User :   more recently yes
User :   they seem to help
User :   but fear of it happening again can affect performance!
Doctor :   I would advice the same, it could be due to the acidic nature of the vaginal fluids , why the inner aspects of the fore skin get affected
User :   but it doesn't always happen during intercourse - it can happen when I'm on my own
Doctor :   ok, do you see any color change or any lesions in the inner skin ?
User :   should I use the lubricant more often to try to make the skin less tight?
User :   not particularly
User :   I have had to treat thrush-like symptoms before now with anti-fungal cremes
Doctor :   yes, using the lubricant more will avoid friction and after some time the fore skin can regain its turgor
User :   it seems to happen when I get a strong erection
Doctor :   What antifungal cream are you using now ?
User :   as if the skin can't stretch enough
User :   none at the moment - I have used casestan duo
User :   my doctor once prescribed Daktacort (?)
Doctor :   ok, do not apply any creams from now , just use the lubricant every time it will help
User :   ok
User :   Do you think they could have made the problem worse?
User :   Does the skin get less stretchy as you get older? I am 47 now
Doctor :   yes , its a fair possibility , where the base in the creams can affect the skin in that way
Doctor :   no , there is no relation with age
User :   I see, thank you - so keep using the lubricant and see how it goes. I do wash under my foreskin every day to keep it all clean.
User :   Just warm water, so soap or anything that might irritate
User :   no soap I mean
Doctor :   do not wash with soap or any detergent materials
User :   No
User :   Just water.
User :   Ok, Doctor, thank you
Doctor :   You are welcome
Doctor :   take care
Doctor :   Bye
User :   Good night
