User :   Do you remember the history of our discussion or should I go over that again?

User :   I have had some abdominal pain, but more concerningly some rectal bleeding and discomfort
User :   bleeding goes back more than 6 months
User :   maybe a year
User :   6 months ago I saw a colorectal surgen that found a skin tag and enlarged papillae
User :   and thought I had fissures
Doctor :   ya i know your history i understand
Doctor :   did you get the ultrasound done
User :   ok good you do remember, I had a lot of tests done now
User :   I went to a new GP
User :   that did blood, urine and rectal exam
User :   all 3 came back normal but he refered me to a Gastroenterologist
User :   so I met with the Gastroenterologist

User :   I mentioned my symptoms and he did a rectal exam and he could find the enlarged papillae, he didn't find much else at the first appointment but scheduled me for a sigmoidoscopy
User :   so I had to do a bowel cleansing prep before it

User :   a lot of inflamation of the lump, soreness...

User :   So yesterday I had the Sigmoidoscopy done

User :   good news is that my colon looked good

User :   I have the report but it is not detailed as I wanted

User :   so colon looked very good and I saw it myself on the monitor
User :   maybe I should still get an ultrasound but lets come back to that

User :   no, he said it was not needed

User :   there were no signs of cancer
Doctor :   were there any masses abnormal 1 s
User :   yes
Doctor :   so can you read the report for me in full

User :   let me get the report, one sec, I believe I know more detail than the report though from what he said and what I saw

User :   ok, it just has a check under findings that says hemmorhoids
User :   which he thought was not an issue but I feel this is very bad to have
User :   I think the prep was part of what caused them
User :   he did not note if they were external or internal though
User :   because I know the treatment for them is different
User :   I asked if they could be removed and he said only if they become a pain
User :   they are very, very uncomfortable

User :   now I feel like I have stool there all the time

User :   which I have already been eating for a few months

User :   yes called piles in most countries
User :   I had no idea that could happen when I agreed
Doctor :   ya i can give you some tips to get rid of it
Doctor :   did he tell you which degree of piles do you have
Doctor :   ya it happens when you are mis diagnosed
User :   no but the way he talked about them sounded like he didn't think it was a problem at all, but I have noticed symptoms and have felt a lump on my right posterior (mostly external) that puffs up under pressure

User :   I didn't get nearly as much detail as I was hoping for and felt too rushed to ask all the questions I wanted to ask

User :   1. Lump that appears on the external spincter from pea sized to large kidney bean size that is sore to the touch, flattens out a minute after BM when the sphincter relaxes

User :   3. Sharp pain of pressure from anus to the tailbone when I sit, sometimes feels like I'm sitting on a baseball

User :   4. Sometimes feels like I have stool or gas in the rectum that won't be releived

User :   1. yes I have been eating a very high fibre diet
User :   even when this all started
Doctor :   2 use sitz bath thrice daily , put luke warm water mixed with the solution and sit in a tub for 10 min
User :   hmm, what kind of solution?
Doctor :   avoid spicy food and use squat toilets
Doctor :   avoid meat for a month or so
Doctor :   till symptoms are relieved
Doctor :   any aneasthetic solution will do
Doctor :   avoid straining in toilets and using laxatives
Doctor :   go for toilets frequently and do not strain
Doctor :   frequent toilet use can decrease the size of the stools and avoid the irritation
Doctor :   regular exercises mild ones
Doctor :   avoid long journeys sitting and walking for a long distance
User :   I have 2 hours of driving each day to get to and from work
Doctor :   avoid using tight clothing and use loose ones if you are comfortable don't wear any inner wears

Doctor :   in that case take a break inbetween and use soft pads underneath
Doctor :   avoid putting hands or irritating the area
Doctor :   maintain high hygienic conditions
Doctor :   take bath regularly
Doctor :   avoid high detergents soaps or fluid for taking bath as these may cause irritaion

User :   I have tried a number of these
User :   Eating a high fibre diet and very low to no meat intake
User :   for a few weeks

User :   lost 20 pounds and am almost underweight now
User :   baths, I have been taking as much as possible
Doctor :   but this time follow my instructions surely u will be benifited and u will be relieved of the symptoms in 3 weeks
User :   but can only afford time 1-2 times a week

Doctor :   don't worry there are many veg meals that serve the same quantity of calories and minerals and vitamins
Doctor :   so if you plan your meals on that i think u will regain your weight
User :   sometimes such high fruit and vegetable intake makes me gassy and large soft stools when can cause problems
User :   I guess I can continue on this plan, but it has not been working so well for me thus far
Doctor :   no thats the reason i told you to use toilets frequently
User :   ok
User :   Breakfast I eat fruit and whole wheat english muffin or oat meal, lunch a sandwich on wholegrain bread with some lean meat or fish like tuna and side salad, for dinner I have salad and vegeatbles and maybe chicken for instance

User :   that is a typical diet for me for the past month or so

User :   In terms of outcome does this only improve symptoms or can this cure the piles so that they would not been seen on next examination?
Doctor :   ya if you follow up the instructions carefully for 3 weeks it will be cured
Doctor :   chances of reccurence are there if you neglect in 1st
Doctor :   and only for final and not responding cases are treated with surgery
Doctor :   and homeopathy is another option
User :   What about rubber banding or infrared treatment? Are these considered surgery?
Doctor :   those are very costly and u need to get it done frequently
Doctor :   but done for only cases not responding cases and final stage cases
User :   High cost is not so much of a concern for me, my health is my primary concern

User :   I want the maimum treatment to prevent recurrance

User :   any recommendations there?

User :   what if I can only manage 1 bath every day or so?
User :   is that still enough to succeed?
Doctor :   homeopathy they use herbal leaves and stuff and they have their natural medicines for curing
Doctor :   ya that is not a problem
Doctor :   but sitz bath you have to practice it for thrice daily
User :   ahh, I don't think this is possible
User :   I work in an office from 8 to 7
User :   add to that an hour before and after for commute
User :   I am very nervous

User :   ok, I will try but am very worried I won't succeed and that this problem could become chronic
User :   I have heard stories where people end up in surgery and are in too much pain for months after
