User :   for the last month or so I've felt an itchy feeling on my skin, sometimes pins and needs, but often a sting, or pin prick (or biting feeling. At first I figured it was an allergy either food or a new soap or something, but don't think so now. Within the last couple of weeks it feels like my hands are shaking more or trembling when I use them. Also feels weaker. It is really scaring me. I am currently taking atenolol for high blood pressure and warfarin for DVT that I had a few years ago. Not sure what is going on with my nervous system or if this is all in the head.
Doctor :   hi
User :   hi
Doctor :   May I know your age and gender?
User :   yes. 38 and male
Doctor :   Since when are noticing these symptoms?
User :   The itchy skin/pin prick feelings on the body have beenh, about a month the shaky/trembling hands and weakness about a week.
Doctor :   Ok
User :   Unfortunately I read stuff online (Web MD) and it freaked me, since a lot of it said MS, Parkinsons, etc
Doctor :   Do you have associated symptoms such as sweating of the body, palpitations, increased appetite,
User :   I seem to be more nervous lately, but it could be the stress from these symptoms. I am a very large man also and have been on the atenlolol for years
User :   no none of that
Doctor :   Are you loosing weight recently?
User :   no. Trying again though, walking, eating right, etc. so have taken off some weight gradually..about 1-2 pounds per week
Doctor :   Ok
Doctor :   Have you had similar symptoms in the past?
User :   no not these
Doctor :   Do you have any other medical illness such as thyroid problems, diabetes, high blood pressure, anxiety in the past?
Doctor :   Do you have family history of parkinson's disease?
User :   High blood pressure being treated with Atenolol. No Diabetes. Had a blood test for suger about 6 months ago and it checked out fine. Nobody in the family had parkinsons that I'm aware of
User :   Had blood clot in the back of my kneed three years ago and still take warfarin
Doctor :   Ok
User :   messed up the valves in my leg, so I wear compression stocks because I travel on business as a consultant
User :   air travel, etc
Doctor :   Do you have difficulties in preforming your daily activities?
User :   no, but I find myself more nervous or concerned about this, thus it makes me more on edge or short with others.
Doctor :   Ok
Doctor :   Let me explain to you.
Doctor :   Based on your description, I do not find any obvious reason to suspect parkinson's disease or multiple sclerosis.
Doctor :   Your symptoms can be explained by somatic causes
Doctor :   secondary to anxiety and stress factors.
User :   somatic?
Doctor :   Somatic conditions are functional disorders that are affected by psychological factors such as stress, anxiety, depression and so on.
User :   I'd really like to stay away from anti stress meds, as I don't think I have any more pressure on me then the next guy, but I might have to talk to my primary physician about this
Doctor :   Apart from this certain medical conditions such as thyroid problems, dry skin, or familial tremors can cause similar symptoms.
Doctor :   Only a physical examination and investigation can reveal if any such medical conditions are associated as the cause.
User :   I thought it might be stress and tried to see if it was worse at certain times (ie before I give a seminar, or under a tight deadline) but found that I would get these symptoms when in a good mood with family or friends
User :   Would there be other triggers or symptons if it were MS or parkinsons. I'm glad to hear you don't think it;s the obvious issue. Reading online med websites doesn't help
User :   I also noticed that if I had a few drinks with buddies or a couple of glasses of wine at a dinner party I didn't have the symptoms
Doctor :   Parkinsons disease is a chronic condition.
Doctor :   Yes, the common presentation is tremors, however
Doctor :   this condition is an extremely rare in young individuals, especially if there are no family history.
Doctor :   More over parkinsons disease is considered if all other conditions have been ruled out.
Doctor :   Since you agree that you have been exposed to stress lately,
Doctor :   I figure that it would be the stress is probably responsible.
User :   Could these stress related symptoms present themselves even when having a good time and not stressed, meaning can physical symptoms caused by stress carry over and not dissappear when there are less stressors?
Doctor :   Yes,
Doctor :   Lack of sleep, improper nutrition, too much work are again stressors.
Doctor :   It doesn't depend on if you are happy.
Doctor :   If you been working hard recently, sleeping less with improper dieting, you are exposed to increased stress.
Doctor :   I suggest you to modify your lifestyle.
Doctor :   Your symptoms should be better with it.
Doctor :   If your symptoms aggravate, then consider consulting a doctor.
User :   When I see my primary doctor are there tests that I should suggest or discuss? I don't want invasive tests, but it would make me feel a whole lot better to know it's not MS or some serious nervous system breakdown.
User :   I imagine a blood work up for thyroid or vitamin definiciency would be in line?
Doctor :   You may need thyroid function test.
User :   I imagine that a blood work up might be appropriate to check for diabeties, thyroid, vitamin definiciency, etc anything else
Doctor :   Have you followed what I have suggested?
Doctor :   MS is diagnosed based on MRI scans,
Doctor :   Further testing will be suggested only if these test show abnormality.
User :   hello?
User :   still there?
Doctor :   Hi
Doctor :   Have you followed what I have suggested?
User :   yes. I appeciate the feedback
Doctor :   Do you have any other query that I can assist you with?
User :   no. Thanks again for your time. It's appreciated. Hopefully, it's just in my head via stress and I can get a better grip, eat healthier, etc
Doctor :   That will be the best way of approaching your symptoms now.
Doctor :   Wish you good health.
User :   take care
Doctor :   Thanks for consulting me.Bye for now
