User :   feeling very depressed and anxious

User :   i've had 3 periods of depression

User :   i'm 40 and male
Doctor :   Since when are you having these problems?
Doctor :   Can you describe your symptoms?
User :   this has been going on since mid january
User :   i've had 8 sessions of ECT and lots of different medication
User :   i'm currently taking lofepramine, buspirone, quetiapine, promazine and zolpidem
Doctor :   Ok
User :   the psychiatrist has said she wants to phase out the buspirone, quetiapine and promazine and give me 200 mg of amisulpride twice a day
Doctor :   Ok
User :   i had another bad period of depression in 2001/2002 and had 12 sessions of ECT and ended up on 100 mg sertraline and 100 mg amisulpride
User :   we seem to be going round the houses with the medication
User :   i don't feel the lofepramine is helping - can you suggest an alternative?
Doctor :   Can you briefly describe your symptoms?
User :   i'm tired all the time, got very bad thoughts racing round in my mind all the time, heart pounding, hot and sweaty, not sleeping well, bad appetite
User :   the sertraline seemed to work quite well at the time - do you think a different SSRI or maybe an SNRI would be useful rather than a tricyclic
Doctor :   Are these symptoms present through out the day or do they appear in intermittent spells?
User :   these symptoms are there all the time
Doctor :   Medical drugs do not give results instantly.
Doctor :   It takes some time before they start acting.
User :   i know - but i've been on the lofepramine for several months now and still feel very depressed
Doctor :   its same with both SSRI and SNRI drugs.
Doctor :   and medical drugs need to supported by good CBT for better results.
User :   the psychiatrist says he thinks i need psychodynamic therapy
Doctor :   Since I cannot examine you personally through this chat, its not legal to prescribe you the drugs here.
User :   that's fair enough - but can you suggest some possible alternatives that i could raise with the doctor here
User :   i've had fluoxetine and sertraline before and brief spells on citalopram and venlafaxine
User :   someone suggested duloxetine
Doctor :   Since i am general practitioner and my expertise on treating depression are limited, I would suggest you to use the second opinion from a superspecialist.
