User : Hi, I have a query on breastfeeding
Doctor : Hi
Doctor : Please proceed with your query.
User : our first kid is now close to completing 9 months
User : he has been feeding on his mother's milk regularly till now
User : in the last month or so, he has developed 4 teeth
Doctor : Ok
User : he tends to bite his mother's nipples now
Doctor : Ok
User : its very painful
Doctor : Ok
User : he has also developed this habit of going to sleep only by sucking milk
User : so, if she withdraws, he creates havoc by crying
Doctor : Ok
User : and not sleeping
Doctor : Is he started on oral feeding?
User : we want to know as to how long he should feed on his mothers milk
Doctor : Is he started on oral feeding?
User : no
User : our paediatrician has said, no milk other than mothers milk
User : he is however eating a few solids
User : as per the paediatrician's advice
Doctor : Ok
Doctor : Let me brief you.
User : so, during the day, he eats solids, in the night, he is on mothers milk
User : and every 2-3 hours once, he gets up and sucks the milk and sleeps
User : it is becoming a problem as the mother is getting disturbed sleep and body pain due to this
Doctor : Its absolutely necessary that you don't feed any thing more than breast feeds till the age of 6months.
User : ok
Doctor : And diet orally has to slowly take over after this.
Doctor : Since immediately stopping breast milk and starting oral diet can cause problems, one has to slowly start them and increase the quantities as the day goes.
Doctor : As the baby grows every day, only breast milk alone cannot satisfy the growth requirements.
User : ok, he seems have a psychology that he needs to suck milk to sleep in the night
User : how do we change this and put him to sleep without necessarily feeding him with mothers milk
Doctor : Its probably habits and insufficient nutrition during the day that is causing him to wake up in the night.
Doctor : Hence I shall suggest you the following measures.
User : please
Doctor : 1. Reduce the amount and frequency of breast feeding.
Doctor : 2. Use pacifiers as an alternative solution.
Doctor : Which too has to be reduced over a period of time.
Doctor : 3. Increase the frequency and quantity of solid food, add good quantity of vegetables, cereals, egg,
Doctor : if you are a non vegetarian, you can add few quantities of meat.
User : what are pacifiers?
Doctor : 4. Make the solid food the main source of diet over a period of time.
Doctor : The simplest form of pacifier are the nipples of the feeding bottle.
User : currently, the solid food is being fed 3 times a day of about 1 cup each with vegetables, cereals etc, 2-3 fruits a day
User : this feeding happens between 9 am to 9 pm in the night
Doctor : 5. You can add other milk like cow's milk and feed via the feeding bottle.
User : after 9 pm, he is only on milk till morning
Doctor : The general principle is to slowly decrease the breast milk and add normal diet to his dietary shedule.
Doctor : Breast feeding can be continued till his age of one and half years,
User : are u suggesting that we start feeding him the pasteurised milk available from the dairy
Doctor : In the mean time, his mother has watch out for injuries, as it can lead to complications.
Doctor : Yes, he is fit to start on pasteurised milk, if there are no lactose intolerance issues.
Doctor : Have you followed what I have suggested?
User : yes i did
User : so, in the night, can we feed him with the pasteurised milk when he goes to bed and also whenever he wakes up?
User : and you are also suggesting that we increase his intake of solid food in the day and the frequencies??
Doctor : If its the first time, that you are feeding with pasteurised milk, I would suggest you to start of in small amounts,
User : is my understanding right?
User : which milk is better - NAN 2 milk or the pasteurised milk?
Doctor : if he tolerating well, and there are no diarrhea or vomiting issues, you can continue with them.
User : NAN 2 is the nestle product which is milk powder mixed in hot water
Doctor : Pasteurised milk would be better than milk products.
Doctor : If you notice any new symptoms such as diarrhea or vomiting, you will have to report to your doctor.
Doctor : Have you followed what I have suggested?
