User :   I have stabbed myself in the gum around tooth with a dry piece of pasta, pain is excruciating and has not decreased in the last 2 hours even after taking paracetamol.

User :   my jaw is very painful
Doctor :   ok , it will persist for a while , i would advice you to take TAB.IBUPROFEN

Doctor :   an OTC IF AVAILABLE
Doctor :   else, you may ned to take a pain killer injection
User :   I have no access to ibuprofen and am in a rural area with no after hours facilities I only have paracetamol
Doctor :   ok , you may wait for some more time , i feel the pain will come down
Doctor :   after 6 hours, you may repeat the PARACETOMOL
User :   I will have to wait to see, otherwise I have to drive for help, thanks

Doctor :   it may be a possibility, but even without that, pain is meant to persist and of greater intensity , since the gum is a very sensitive area
User :   will an ice pack help

User :   no swelling
Doctor :   you may try it, generally for swelling ice packs help
Doctor :   i suggest you rest , and avoid chewing and speaking for a while
Doctor :   since you have taken pain killer, it needs time
User :   okay, thanks i'll have to see the night out and if still bad in the morning go to a medical centre, thaNKS, goodbye
