Doctor :   hi
Doctor :   please proceed with your health query
User :   i thick you were the one a talked with yesterday
User :   rim enhancement in follicles
Doctor :   yes, I remember.
User :   i dont know what happened yesterday but we were cut off
User :   doctor i really am worried of the MRI report
User :   having seen lymph nodes in MRI is that normal?
User :   what lymph nodes should I be worried
Doctor :   seeing lymph nodes are normal. lymph nodes are essential part of body. if they are enlarged or inflamed then they are abnormal findings on MRI
Doctor :   as I had said earlier you will need biopsy or FNAC of the lymph nodes will be conclusive to say if something is serious or not.
User :   tiny bilateral iliac chain and inguinal lymph nodes are identified
User :   oh right yes
Doctor :   and the MRI report needs to be interpreted by another radiologist to see if the interpretation given earlier was right (esp about the metallic particles)
User :   I forgot to mention yesterday my right breast becames first very itchy then like after a day it becames swollen heavy and painfull what could this be
User :   and then in the middle of my chest even this very moment there is like pressure, like something is pressing it
Doctor :   ok. is this your probable ovulation time?
Doctor :   is there any discharge present from the breast?
User :   no because it happenes 2-3 times a month
User :   none but there is a hard lymp in the breast sides near the armpit
Doctor :   ok
User :   it only becomes painfull when touched
Doctor :   is the lump freely movable inside? or does it seem fixed?
User :   fixed
Doctor :   how is the skin above the area of lump?
User :   doesnt move and when pressed it becomes painful
Doctor :   is the pain more in the area surrounding the lump?
User :   no pain is just in that lump
User :   i noticed it like sept of 2009
Doctor :   ok. was anytime a mammogram done?
User :   no
Doctor :   since the pump is fixed and with pain, i would strongly recommend a mammogram to be done to make sure it is not a cancerous lump
User :   i ask the gynecologist if i should get one she said that its not needed
User :   ok
Doctor :   did you follow what I have said?
User :   with the lump in my right breast and the follicle with rim enhancement in my right ovary is there a connection?
Doctor :   it only possible to say if there is connection or not after the mammogram and FNAC from lump. if the FNAC and the right ovary findings and lymph nodes findings are similar then it can be proved that they are all interlinked.
User :   and everyday for how many hours i would always feel dysmenorrhea like pain in the right pelcvic area only
Doctor :   ok.
User :   ok how does the FNAC procedure done?
Doctor :   FNAC is fine needle aspiration cytology. A biopsy needle is inserted under ultrasound guidance if structure to be biopsied is deeper, and celels are aspirated to be studied under microscope
Doctor :   to identify the cell type and nature of problem.
User :   is this done under anesthesia?
Doctor :   yes. under anesthesia if deeper , or local anesthesia for superficial ones
Doctor :   is there anything else I can help you with?
User :   last question doctor
User :   does nabothian cysts disappear on its own?
Doctor :   sometimes they do if very small. if not, then they need to be treated with cryotherapy or excision
User :   i have 3 nabothian cysts that never goes away and increasing in size for the past 5 years is this something to be worry iof
User :   my pap smear results the last time is severe inflammation reparative changes
Doctor :   if they increasing in size and numbers, get them checked, they can turn cancerous anytime if left alone over long periods of time, due to resultant chronic cervicitis and erosion
User :   but im wondering why gynecologists fropm here always telling me that that is normal'
Doctor :   when was your last PAP smear tested?
User :   mar4ch 290,2010
Doctor :   nabothian cysts are normal , but when increasing in size and numbers over short periods of time, then they are not normal
User :   3 cysts are always in the same location
Doctor :   yes, they will be in same location once they have appeared, they do not change their positions
User :   from 5mm. seen last 2007
User :   .8cm june of 2007
Doctor :   ok
User :   1cm oct of 2008
User :   .6cm feb of 2009
User :   and on march 2009 the report was showing a lobular anechoic structuire of about 15.9mm size w/in its posterior upper segment
User :   a smaller nodule of about 4.8mm size is noted superiorly
User :   small low echogenicity foci are noted w/in its outline
Doctor :   there has been progressive increase in it. have you discussed about getting a colposcopic examination done?
Doctor :   it wil show if the cervical changes are normal or abnormal
User :   no because they always say that it is normal
User :   they let me just did a pap smear
User :   what could this be?
Doctor :   would you like to take a second opinion for all your problems by our panel gynecologists?
User :   sure
Doctor :   i meant about your ovary issues, lymph nodes, breast lump, nabothiann cysts?
Doctor :   ok. i will help you in this.
User :   that would be great
Doctor :   you can see as recommendation on your right hand side of chat window.
User :   but doctor in your own opinion what would be this
Doctor :   yes, you need to tell your detailed history of each year and attach your reports copies along with MRI scan images (films)
Doctor :   to get a good second opinion on what is going on and what should be your next steps
Doctor :   As I said, you will require a detailed investigation by FNAC for lymph nodes and breast lump, and colposcopic examination and repeat PAP smear for cervix.
Doctor :   based on these we can say if they are related and if anything is serious
Doctor :   I have to close this window since there is no response from your side, we are available 24/7 please get back to us if you need further medical advice.
