User :   i have a pain that starts in my right abdomen and goes down the front of my right leg. I have had numerous visits to the hospital and have had CT, ultrasound, MRI, blood test, urine tests, etc. etc. and nothing has shown as an issue. I am now seeing an osteopath as i was told it could be a muscular/nerve problem. I have also noticed that when I drink alcohol, the pains get worse.
Can you help??
Doctor :   Hi
Doctor :   I am Dr. XXX
User :   hi
Doctor :   Let me read the query
Doctor :   What is your age?
User :   38
Doctor :   What is your occupation ?
User :   XXXXXX
Doctor :   What is the severity of the pain on a scale of 1 to 10?
Doctor :   Is it continous pain or intermittent?
User :   it varies but at the its worst its about a 7
User :   the pain is usuall continuous and down the leg it is more of a dull ache
Doctor :   When was the first time you experienced the pain and can you detail where exactly it starts in the rt abdomen?
User :   yes it started in the abdomen just below the ribs and has been about 6 months. At first my doctor thought it was kidney problems but nothing has shown up in any tests
Doctor :   How long does the pain stay when it starts and does it immediately radiate to the rt thigh or takes some time?
User :   the pain is there constantly but it does seem to get worse (especially in the abdomen) when I drink alcohol. I wouldn't class myself as a heavy drinker though
Doctor :   Since how long have been consuming alcohol and how much per day?
User :   i have drunk alcohol since my teens but only once/twice a week
Doctor :   Was the Ultrasound of the abdomen normal, any findings of liver or gall bladder? like fatty liver?
User :   no problems with ultrasound
Doctor :   As this is an online consultation, please be patient to answer my questions
Doctor :   Till what extent in the thigh the pain radiates?
Doctor :   upper 1/3rd or middle or lower 1/3rd of it
Doctor :   Tell me which third of the above?
User :   the pain is usually a dull ache but feels like a slight tingling sensation. Its definitely not a sharp pain it is more in the lower 3rd
Doctor :   Which medicines did you use yet?
Doctor :   Were Vitamin B12 and Vitamin B1 levels tested in the blood?
Doctor :   Is there any pain on movement of the body like bending or stretching back or sitting?
User :   I had two types of antibiotics as originally my GP thought it could be a kidney infection. Other than that i've had no other medicine. I haven't had vitamin levels checked. I do have a back pain further up by the shoulder blades
Doctor :   Does the abdomen pain increase with forward bending or sitting or backward stretching?
User :   i'm not aware that it does
Doctor :   Does blood profile indicate any large redblood cell?
Doctor :   called macrocytic or any abnormal cells in the blood picture?
User :   no, recent blood test was normal
Doctor :   I do not know if your doctor was keen in knowing the complete blood picture with peripheral smear to know the nature of RBC which can be of some help
Doctor :   Taking all the possibilities I could think of these
Doctor :   1) Fatty liver with Vitamin deficiency that can be neurological
Doctor :   2) If an endoscopy was done, gastritis can be ruled out, as the alcohol intake increases the pain and the pain can be referred
Doctor :   3) I also suggest you discuss the option of putting you on trial ' PRAZOLES' for a month with your doctor
Doctor :   4) Alcohol intake hampers the treatment and recovery
Doctor :   hence it should be curtailed asap
Doctor :   A good physical examination can narrow down the diagnosis and helps in better treatment
Doctor :   I guess the liver function tests are normal
Doctor :   Since the kidney problem is ruled out by your doctor I do not emphasize its retest
Doctor :   Did you follow?
User :   yes thanks
Doctor :   Do you have any more questions?
User :   no - thankyou
Doctor :   Thank you for consulting me today
Doctor :   Hope for a fast recovery
Doctor :   Bye for now
User :   thanks
