User :   Hi Dr, I am aged 29,weigh 63 kg and my height is 5'10".I am working in IT. I had an operation for anal fissure 7 years back(Not laser operation). From yesterday afternoon,i am having some pain in the anus area.I haven't yet seen a doctor till now.Will consult soon. No pain or bleeding during defecation. But,after some time i am having this pain.Yesterday night,applied an ointment pilex for this which i bought in the local medical shop.I have normal bowel movements only. Now,my query is: 1. Any chances of recurrence of anal fissure again? 2. If yes,can it be operated.If yes,how long will it take for recovery (assuming its a laser operation) Please suggest.

User :   But day before yesterday.. it was bit hard.. could that be the prob?

Doctor :   From sitting here, without examining you, I cannot pinpoint the cause/chance of fissure recurrence.
Doctor :   However since your report of passing hard stool and developing pain following this, there are chances
Doctor :   that there could have been a trauma(mild).
Doctor :   and yes, a small chances of fissure development.
Doctor :   Since your symptoms is a only a few days old(just about a day),
Doctor :   need for surgical treatment at this point is doubtful.

Doctor :   I suggest you to continue the use of local antiinflammatory analgesic creams/ointments such as pilex.
Doctor :   Along with this, you can also use
Doctor :   warm/hot water sit baths twice or thrice a day for the next 2-3 days.
Doctor :   Avoid straining while defecation.
User :   Ok
Doctor :   You can use a small dose of laxative such as Cremaffin syrup ( 1-2 teaspoon) at night.
Doctor :   Drink plenty of water and use plenty of fibres in your diet.
User :   hot water just plain hot water??
Doctor :   Yes, just plain hot water.
User :   How long can i continue with this creams Dr??
Doctor :   Let it not be too hot,

Doctor :   so that you don't injure yourself.
Doctor :   You can continue its use for a few days.
Doctor :   I expect your symptoms to reduce in the next few days.
Doctor :   however, if your symptoms are persisting, getting aggravated,
Doctor :   or you notice that there is profuse bleeding while passing stool,
Doctor :   or a mass noticable while defecation
Doctor :   you will need to consult a doctor for a per rectal examination.

Doctor :   The per rectal examination will conclude what the cause is and if you will need a repeat surgery.
User :   and about the creams is proctosedyl better than pilex??

User :   if suppose a surgery is required.How long do i have to take rest before getting back to work?
Doctor :   Pilex is an ayurvedic product, known to contain antiinflammatory properties.

Doctor :   as a medical doctor practising allopathy, my knowledge with regards to ayurveda is limited.

Doctor :   proctosedyl doesn't have antiinflammatory properties.
Doctor :   Hence, if pilex is helping you, you can use the same.

Doctor :   Hot water sit baths will provide you with better pain relief and antiinflammation.
User :   pilex is just ok..not very good. but the pain has subsided a bit

Doctor :   You can use the pilex, then with the hot water sit baths.
User :   Also,can you suggest some good surgeons in chennai for this if you have any idea?
Doctor :   Surgery, if necessary is not a major one.
Doctor :   You will need absolute rest for a day or two.
User :   it will be a laser operation right?
Doctor :   Followed by dietary and activities restriction for a week or just over it.
Doctor :   Both conventional surgery and Laser are available.
Doctor :   The examining doctor will be the best person to decide on the type of surgery that is needed.
User :   I prefer Laser,as the pain will be less & can get back 2 work soonn
