User:i need to talk to an endocrinologist
Doctor:this is Dr XXXXXXX
User:hi Dr XXXXXXX
Doctor:i am a general physician and i think i will be able to help you
User:i need to talk to an endocrinologist
Doctor:if i find a need, I can discuss your case with an endocrinologist and get back to you
User:this is very thyroid and harmone related question
Doctor:Thats fine
Doctor:Please go ahead
User:is it not possible to talk to an endocrinologist directly?
Doctor:You can post a query on Ask a specialist
Doctor:There an endocrinologist can directly address your query
User:i hv problem which is a combination of PCOS, Insulin Resistance and hypothyroidism
User:how do i treat all the 3?
Doctor:Generally speaking, hypothyroidism is treated with Eltroxin
User:i hv been asked to consume melmet (metformin), which i previously did and it took care of all the 3 probs i hv
Doctor:Which is thyroxine hormone
User:i was asked to stop it later as i had become hyperthyroidic
Doctor:For PCOS and Insulin resistance, weight loss and MEtformin is recommended
User:now, after 6 months, am hyppothyroidic again with PCOS n insulin resistance
Doctor:The dose needs to be carefully titrated after seeing your reports
Doctor:What dose of Eltroxin were you consuming?
User:apparently, metformin takes care of all the 3 probs i have
User:i hv never consumed eltroxin
User:i was always only on melmey
User:and that took care of all the problems
Doctor:Then what we need to do is get you to lose weight initially
Doctor:Metformin will not cause hyperthyroidism
Doctor:What is the dose of Metformin that you were taking?
User:1000mg after breakfast and 1000mg after dinner
Doctor:We can bring down the dose to 500 mg twice a day and try then
Doctor:But this needs to be done after you have seen a endocrinologist again
Doctor:Also, losing weight will help a lot
User:also, few years ago, i was hyperthyroidic n PCOS n insulin resistance. i was adviced melmet n it took care of all my probs. later i stopped it n these issues again cropped up
User:again, all these probs were subsided n i stopped melmet
Doctor:There is no permanent solution actually
User:n now again, after stopping the melmet for 6 months, these probs hv again cropped up
Doctor:Its a long term problem and you would need to stay on treatment
User:oh :(
User:is the treatment for life?
Doctor:Not necessarily
Doctor:But Insulin resistance and PCOS can become a lot better with weight loss
User:i knw tat.. but, coz' am hyperthyroidic, losing weight is 3 times more difficult than the normal person.. any amount of dieting and exercise takes a LONG time to hv an effect on me
Doctor:I understabd
User:n does losing weight help in gettin hypothyroidism to its normal range?
Doctor:A dietary modification and exercise regimen is what you need
Doctor:I need you to exercise for atleast 50 minutes daily
Doctor:Thats a brisk walk, say covering about 6 km in 50 minutes
Doctor:For the dietary modification, I would need you to eat a heavy breakfast and light dinner
Doctor:You must eat every 3-4 hours
Doctor:Also, limit the fat and sugar
User:the range is b/w 0.3 - 5.5 and mine is 8.15
User:6 months ago it was 0.02
Doctor:This is the TSH value
Doctor:It definitely needs to be treated
User:this is wt determines hyper n hypo thyroid rt?
Doctor:A high TSH means hypothyroidism
User:my doc said, since its varying, he wants to observe me for 2-3 months more
User:if it increaes further, he wil put me on medication
Doctor:I think it needs to be treated
Doctor:But you can always ask an endocrinologist
User:apparently, according to him, for a few, it gets back to normal by itself
Doctor:You post your query and reports and can directly ask him
User:n am hoping it does.. coz' if i start the thyroid medication, its for life!!!!!!
Doctor:A second opinion is always useful
Doctor:I understand
Doctor:But I would rather treat you for something that can be XXXXXXX
Doctor:than leave you untreated
User:how XXXXXXX hypo thyroidism can be?
Doctor:It can lead to heart disease, high cholesterol
Doctor:It can cause depression which spoils the quality of life
User:ya.. i any which way feel low frequently
Doctor:Thats why we need to treat it in my opinion
User:alright.. thanks Dr XXXXXXX
User:i will consider seeing another endo and seek a 2nd opinion
User:thanks again
