User :   Yes
Doctor :   May i know your age?
Doctor :   Since when you have this?
User :   Nearly 50 Years
User :   for the last 4 years
Doctor :   Do you have regular swelling on face when you wake up?
User :   I do not have swelling on face but only in legs. Recently, it has flared up

User :   As far as I know, I have not been taking sterioids. I do not have diabetes or high BP. My BP is usually in th erange of 130-80. However, I have Ankylosing Spondylitis for the last 23 years
Doctor :   Specific treatment of nephrotic syndrome depends on the disease's cause.
User :   The swelling in legs satrted only 4 years ago then it was diagnosed with nephrotic syndrome
Doctor :   ok
Doctor :   Are you on any medicines now?
Doctor :   do you take diuretics?
User :   Yes on Dytor 20 and Zytanix 2.5. My HB is low around 9.5 to 9.9. I have been taking weekly injections of Neo recrmon 4000
User :   previously, I was taking dytor 5mg only.
Doctor :   dytor i s fro increasing urine flow
Doctor :   are you on diet control?
User :   I have been asked to avoid high Potassium fruits and also restrict diet of Pulses
Doctor :   It's often a good idea to reduce the amount of salt (sodium) in your diet if you are diagnosed with certain conditions, such as nephrotic syndrome
Doctor :   For patients with nephrotic syndrome, their diet should provide adequate energy (caloric) intake and adequate protein (1-2 g/kg/d). Supplemental dietary protein is of no proven value.
User :   The salt intake is low. However, the doctor prescribed K-Bind for a week when Potassium was high. After this potassium came within limit but swelling started again
Doctor :   how is your blood cholesterol level?
User :   low
Doctor :   ok
Doctor :   There are no activity restrictions for patients with nephrotic syndrome.
Doctor :   Ongoing activity, rather than bedrest, will reduce the risk of blood clots.
User :   due to swelling i am not able to walk much and i also feel breathless
Doctor :   IF you dont find relief with dytor
Doctor :   you may ask to your dr about, salt-free albumin given through a vein (IV). Albumin helps remove extra fluid from the tissues.
Doctor :   Fluid reduction should occur slowly to avoid further kidney damage and low blood pressure.
User :   I have been advised to have intravenous injection of Encifer
Doctor :   in case you feel btreatless
Doctor :   breathless
Doctor :   then injection of medicines might be required
Doctor :   to reduce the fluid overload
Doctor :   Are you measuring the input and output of fluids
Doctor :   ok
User :   No
Doctor :   encifer is iron supplements
Doctor :   to increase your hemoglobin levels
User :   I feel breathless only when I walk more than say 50 yards.
Doctor :   ok it might be due to the fluid overload in heart
User :   My basic question is whether this condition is treatmentable
Doctor :   it is not fully curable
User :   Can it be managed
Doctor :   only supportive measures are available
Doctor :   yes it involves diet control ,medicines
Doctor :   and supplements
Doctor :   like calcium iron
User :   does this condition have bouts of attack , Because I was quite managing for the last 2 years with low HB, low albumin and no swelling, no breathlessness
Doctor :   Cyclophosphamide, cyclosporine, or mycophenolate mofetil, when treatment with corticosteroids is not successful.
Doctor :   yes it will
Doctor :   First treatments can last from 6 to 15 weeks, often longer in adults. Depending on how severe your symptoms are or whether they return, ongoing treatment may be necessary for months to years, or even for the rest of your life.
User :   I believe that I am only taking diuretics and calcium and viatmin supplements
Doctor :   ok
Doctor :   now you will need iron too
Doctor :   since hb is low
User :   Iron medicines have not worked / last time also intravenous encifer raised HB to 12
Doctor :   ok
Doctor :   it is because of poor kidney function
Doctor :   kidney helps in hb production
User :   any other prescription or suggestion
Doctor :   you might need some immunosuppressives like
Doctor :   cyclophosphgamide etc
