User:Hello Good Morning Doctor.
Doctor:Good morning.
User:I would like some advice on acne and scars please
Doctor:Can you brief me a bit more?
Doctor:Since when do you have the acne problem?
Doctor:Are you on any treatment for it?
User:sure. Both my parents do not have a history of acne
User:however for some reason i do. Ive gone through numerous treatments since teenage-
User:like dr batra, english medicines , baksons etc
User:to no avail.
User:now i do not get acne on my face , however it still appears on my back only the area below my neck and central back only
User:ive tried a lot of creams applied medicines, sandalwood paste but it doesnt go.
User:i do have dandruff as a result of harsh chemicals and hair straightening as well, , even that doesnt seem to go. I assume its because of Dandruff as well partially.
User:i use Nizral 2% then only my scalp clear. the minute i stop using the shampoo the dandruff is back.
User:and then acne flares
User:i really dont know what to do . please help me out.

User:yes. thats when the doctor prescribed Nizoral 2% every alternate day.
User:I have also been [rescribed medines for the acne on my back by some of the best dermatologists in bangalore
User:but it seems stubborn . the marks dont go and just before my period , it flares up again.
User:Its like boils and pimples and they leave a nasty black XXXXXXX as a result of which i cannot wear clothes with a XXXXXXX back .
User:Is there any permanent solution to this doctor?

Doctor:Acne is a recurring clinical condition that is associated with multiple factors.
Doctor:Commonly in women they are related to hormonal changes.
Doctor:Hormone changes that occur with periods are known to produce flaring of this acne.
User:Ok , is it also related to the fact that i recently got married in Dec 2010. But i have never had any contraceptive pills etc .

Doctor:However stress factors can play a role.

Doctor:As far as treating it is concerned, it needs proper medication, a healthy life style and managing stress properly.
Doctor:There are various medical drugs available to treat this.
Doctor:most commonly used are isotretinoin.
Doctor:This is available as oral drug as well as topical creams.
Doctor:They are used most frequently by doctors.
Doctor:If you haven't tried this one, you might try it undersupervision of your doctor.
Doctor:In addition sometimes resistant cases may need a course of antibiotic for few months.
User:I am hardly stressed doctor. I am a working woman but i do not have stress I have a good work life balance .
User:Alright - is isotretinoin an ointment?
Doctor:it is an ointment.
Doctor:You may try it if you haven't used it earlier.
Doctor:it is available in the market as 0.025% and 0.05% creams.
Doctor:Your physician can prescribe it to you.
Doctor:In addition, avoid washing your back frequently,
Doctor:avoid any harsh chemical products.
Doctor:Proper hygeine, good and regular sleep, regular diet will help.
User:Should i use a loofah while having a bath? Right now i just use soap and water
User:i sleep 9 - 10 hours a day.
User:diet - should i avoid chocolates and oily food and icecreams?
User:will the scares go with the cream you have prescribed Doctor?
Doctor:Chocolates, high calorie food can influence hormonal changes that can affect acne.
Doctor:So it would benefit you to avoid them.
User:should i exercise to release excess heat in system?
User:when i used to swim the acne reduced

Doctor:Swimming, physical exercises do not have any direct association with acne, but they do have a significant role in managing the effects of any stress on the body.
User:ok. When i used to swim it greatly reduced the acne and also the scars.
Doctor:I would suggest you to try these medications, involve in stress reducing techniques like physical exercise, meditation, yoga, swimming and so on.
Doctor:It would help.
Doctor:As far as scars are concerned, these scars usually fade away with time.
Doctor:if the scars are persisting, you may need a different medications.
Doctor:Depigmentation products like mederma have helped many users.
Doctor:You can try them on very dark, cosmetically disfiguring pigmentation.
Doctor:Alternate method of treating dark spots secondary to acne would be by a simple procedure called dermabrasion.
Doctor:This can be done at your dermatologist's office.
Doctor:1-2 settings of this would clear off most pigmented spots.

Doctor:Effective depigmentation products contain one or all of these ingredients - vitamin D or aloevera, kojic acid, hydroquinone, etc
Doctor:In addition there are a few herbal ingredients that may help (which a homeopath or ayurveda practitioner can suggest you).
Doctor:You can try any of those products.
Doctor:If you choose to use isotretinoin cream, avoid sun exposure when the cream is on your skin.
Doctor:It can increase pigment formation.
Doctor:I understand you are facing a difficult problem.
Doctor:Treatment for such resistant and recurring problem
Doctor:is going to be slow and gradual.
Doctor:Be patient while using any medication or depigmentation products.
Doctor:If none of the products help, you can try the dermabrasion procedure sometimes.
Doctor:Now to your last problem, dandruff.
Doctor:Dandruff is caused by a fungus which commonly inhabitates the scalp and sometimes other parts.
Doctor:Antidandruff shampoo can treat it effectively, but as this is a fungus
Doctor:there is always a chance of recurrence.
Doctor:Since nizaral shampoo works best for you.
Doctor:Do not change it.
Doctor:use it to shampoo your hair atleast twice a week.
User:if i use onion paste on my hair will it work?.
Doctor:I am sure with regular usage for 6-8 months, this problem can be reduced considerably.
User:i ve been using Nizral since 4 years . As long as i use it there is no dandruff- the minute i stop Dandruff is back
Doctor:I am not aware of the effectiveness of herbal ingredients including onion pastes on this fungus.
User:is there a permanent solution to this fungus? is it because of the water i use for bath?
Doctor:As there are no scientific evidence that prove the effectiveness of onion paste here, I would not be the right person to comment about it.
User:I read it in a magazine- actually ii am so fed up of this dandruff ive tried everything from beetroot paste to onion to Nizral to all product shampoos in the market .
Doctor:Reduced personal hygeine is one factor that is commonly associated with this fungus infection.
Doctor:Apart from this any chemical or harsh product that damage the scalp can increase the chances this infection.
Doctor:So avoid using multiple chemical cosmetic product or harsh water.
Doctor:The effectiveness of nizoral is proved to be better when this shampoo is used on the scalp (not just the hair) and left for few minutes before washing.
Doctor:I would encourage you to do this.

User:I should leave it for 5 minutes on hair?
Doctor:Rub the shampoo on the scalp and not just the hair
Doctor:and leave it for for 3-5 minutes before washing.
Doctor:This increases the effectiveness of this shampoo.
Doctor:I suggest you to try this method of usage, twice or thrice weekly for 2-3 more months and let me know the results.
