User :   HI Doctor. Since I was six months pregnant, I have had a nasal congestion, a runny nose and migraines. Now five e years later the symptons are the worse with all of the above, but I also get migraines, cannot smell or taste food especially in cold weather. I live in a climate where its mostly hot and humid but I wake up every morning as if a bomb has exploded in my head and am mostly fatigued and have a runny nose. I work as an accountant and find it very hard to concentrate and if I am in airconditioning the congestion is worse. I have tried antihistamines, nasal sprays and saline solutions. Its all just temporary relief. I am very fatigued at times and find it hard to take care of my four year old daughter when I am so tired. I am also getting depressed from all this. Is there anything medically that can be done to rid me of all this discomfort ? Thanks doctor.
Doctor :   Hi
User :   hi doctor
User :   ok thanks
Doctor :   Ok
Doctor :   it looks like u have severe persistent allergic rhinitis
User :   I have been skin tested and have no allergies. A doctor diagnosed me as having non allergic rhinitis. i have since researched and i think i have vasomotor rhinitis.
Doctor :   ok
Doctor :   Have you consulted allerhy specialist
User :   yes i have at the hosptial here and the doctor skin tested. he has made the diagnosis that i have non allergic rhinititis
Doctor :   ok
Doctor :   so he concluded it is vasomoto
User :   i have never had any of these symptons until i was 32
Doctor :   ok
Doctor :   Ipratropium is ideal for patients presenting with only rhinorrhea. It is best used in combination if patients present with rhinorrhea and other symptoms because it possesses no activity for treating or preventing sneezing, itching, or nasal congestion. Clinical studies showed that ipratropium reduces the duration and severity of rhinorrhea by 33% and 29%, respectively, compared with placebo.
User :   no he just said that i had non allergic rhintitis. but i researched it on the net and the i have the exact symtpons of vasomotor rhinitis
Doctor :   This is avaoalable in nasal spray
Doctor :   ok
Doctor :   The treatment is almost same
User :   is there some kind of surgery that can eliminate this all together because the medications are only temporary relief
Doctor :   Treament spins around symptom control
Doctor :   if there is Devraited nasal Septrum in the nose ten the surgery will help
Doctor :   but it is medical problem
Doctor :   so medications will help
Doctor :   Nasal corticosteroids follow the same pathway of reducing inflammation as that of medications taken systemically, but their anti-inflammatory activity is localized to the upper airway.
Doctor :   Antihistamines are useful in relieving rhinorrhea, sneezing, and nasal pruritus.
Doctor :   Pseudoephedrine and phenylpropanolamine, systemically administered sympathomimetics were recalled from United States market.
Doctor :   Surgical therapy should be considered a possible adjuvant to continued medical therapy. Intranasal examination may demonstrate a deviated septum and/or pronounced turbinate hypertrophy.
Doctor :   so Get ENT examination
Doctor :   Also get CT sinuses
User :   well i found that when i went to the US for a holiday, i felt like i was going to die because it was winter and the antihistimines werent working
Doctor :   if there is hypertrophy of turbinates
Doctor :   ok
User :   i couldnt taste or smell for three weeks and the migraines were quite severe.
Doctor :   ok
User :   the symptons are getting worse so maybe i will go to a doctor here and request a ent examination and a CT sinuses scan.
Doctor :   excellent
Doctor :   that is good move
User :   Ok thanks doctor. takecare
Doctor :   use all -antihistamines, nasal steroid sprays, XXXXXXXX psuedophedrine
Doctor :   all these -even if they are temporary, will give u relief
Doctor :   Thank you
Doctor :   Bye
