User :   I get very sore breasts for about a week a month, I also become extremly short tempered and fly of the handle at stupid thing. I suffer from really bad fatigue at this time to the point where getting up and dressed is all I can do. I have had all the thyroid tests etc and bloods came back clear, any ideas

Doctor :   may i know your age, weight , height ?
User :   age 36, 10st 4, 5ft 7
Doctor :   are you on any medications ?
User :   none
Doctor :   have you done any blood tests ? other than the thyroid ?
User :   cotisol, Vit D, glandular fever,a few can't remember all.
Doctor :   and all the tests were normal ?
Doctor :   any serum electrolytes done ?
User :   all normal the only thing was slightly low hemaglobin but red/white count all ok
Doctor :   in that case, the fatigue is due to anemia
User :   they say i'm not amenic as red count good?
Doctor :   MAY i know the HB count
Doctor :   if the Hb is low, then it is anemia, independent of the RBC count , as the counts can change over the day
Doctor :   the Hb has to be above 12

Doctor :   it is ok, if the Hb is above 11.5
User :   not sure what HB stands for rbc's 4.33 10*12/L if that makes any sense

User :   soory mistake made serum globulin low, 23.1 g/L
Doctor :   thats not Hemoglobin
Doctor :   the other condition that can present in this way, if all the blood tests are normal, is Chronic fatigue syndrome
Doctor :   Chronic fatigue syndrome is a condition of prolonged and severe tiredness or weariness (fatigue) that is not relieved by rest and is not directly caused by other conditions.
Doctor :   Other factors such as age, prior illness, stress, environment, or genetics may also play a role in this condition.
User :   have looked into that and from what i've read seems to be all the time, this seems to come in cycles

Doctor :   and how long have you had them this time around?
Doctor :   do these symptoms come during the time of your monthly periods ?
Doctor :   and do the symptoms go away with rest ?
User :   every month which made me think hormone related maybe? been on coil for 5 years, symptoms satrted year and half ago. symptoms to not go with rest.

User :   between 4 - 10 days
Doctor :   do u have any breathlessness, joint aches ?
Doctor :   difficulty walking steps or uphill ?
User :   no but do get musle aches, like i've over exercised

Doctor :   any swelling in the legs ?
Doctor :   that go away with rest or limb elevation ?
User :   no swelling
Doctor :   Have any tests like X rays and echocardiogram or EKG done ?
Doctor :   has a treadmill test done ?
User :   no
Doctor :   so no tests have been done ?
User :   none other than bloods.
Doctor :   is your BP normal ?
User :   low BP
Doctor :   ok
Doctor :   may i know the last reading ?
User :   don't know it but always commented on that it was low
Doctor :   ok
User :   before this fit and healty played lots of sports etc, good idet etc
User :   thats good diet
Doctor :   did you have this low BP within the past year or even before this time ?
User :   before
Doctor :   since how long have you had a low BP ?
Doctor :   DO They coincide with onset of tiredness and other symptoms
User :   not sure only really got it testedfor bp when had coil check up etc low before this kicked off

Doctor :   so now there are few conditions that i would like you to get checked for
Doctor :   1, check if your Hemoglobin is normal , if its low, you may be having anemia
Doctor :   2. check your BP, if its still low, then medications can be given to improve your vascular tone to improve your BP
Doctor :   3. GET a stress test (treadmill) to check if any issues are there with the heart, and its function
Doctor :   4. Get a Pulmonary function test or a Xray chest to rule out any lung conditions affecting your oxygen intake from air
Doctor :   5. If all the tests are normal, then you definitely have Chronic fatigue syndrome
User :   Any treatment for CFS ?
Doctor :   a healthy diet + low dose tricyclic medications + physiotherapy
