User :   Hi, I am 19 yrs old and a female. My left breast only has been itchy for the last week and a half and it seems to be peeling. Why is that?
Doctor :   Hi
Doctor :   How can I help you ?
User :   I was wondering why only my left breast has been itching for almost a week and a half. It seems to be peeling a bit too..I am not sure what is causing it.
Doctor :   Ok. May I ask you a few more questions pertaining to your problem ?
User :   That's fine.
Doctor :   Do you see any lump in your left breast or any hard or soft swelling or any Nipple retraction ?
User :   No there is no lumps and neither is there swelling at all. It seems to be just really itchy in the tip and the surrounding area of the tip, I have a small flake and that's the only one that I have.
Doctor :   Are the flakes reddish or pinkish in colour ?
User :   They are both the same color and the only difference that you can see is that on my left I have the small flake that wants to peel but it hasn't yet. The top skin is somewhat black/ dark brown the same color of my nipples and the skin that is reveal under it is really really light pinkish almost a light brown
Doctor :   Ok.
Doctor :   May I know your height and weight and May I know if you sweat a lot , moist skin or are you having a dry skin ?
User :   My height is 4'11'' and I weight 137. I do not sweat a lot, but I tend to have dry skin during the winter time which is typical for everyone. I have always put lots of lotion all over my body during this time, but its the first time one itches and started to peel.
User :   In case it might be helpful my left breast is my bigger breast.
Doctor :   Please do not worry, Is it 137 pounds or Kiolograms ?
Doctor :   Ok.
User :   Sorry, its pounds
Doctor :   Thats ok.
Doctor :   Now let me tell you that itching sometimes is caused by very simple reasons and the most important reasons are : Doctor :   1. dry skin,
Doctor : allergy,
Doctor :   3. irritation,
Doctor :   4.Intertrigo
Doctor :   For all these the treatments are pretty easy.
Doctor :   But I would also not rule out Paget's disease, if you have any more symptoms developing sooner, otherwise there is no need to worry.
Doctor :   Hope you have followed.
User :   So, based on my description of my nipple flaking a bit, I shouldn't worry at all. Its not cancer or anything like that.
User :   What other symptoms should I look out for, for Paget's disease?
Doctor :   Only itching and one or two flakes do not conclude a Paget's or Inflammatory breast cancer (IBC) .
User :   Can you explain what intertrigo is?
Doctor :   An intertrigo is an inflammation (rash) of the body folds,An intertrigo sometimes refers to a bacterial, fungal, or viral infection,A frequent manifestation is Candidal intertrigo.
Doctor :   An intertrigo usually develops from the chafing of warm, moist skin in the areas of the inner thighs and genitalia, the armpits, under the breasts
Doctor :   Intertrigos occur more often among overweight individuals or in breasts if they are large and develop at the folds between the chest wall and the breast.
User :   I think I was able to followed through of what an intertrigos is.
Doctor :   Ok
Doctor :   Now about other symptoms of Pagets disease.
Doctor :   1.Early symptoms may cause only mild irritation and may not be enough to prompt a visit to the doctor
Doctor :   More advanced disease may show more serious destruction of the skin (1). At this stage, the symptoms may include tingling, itching, increased sensitivity, burning, and pain. (2.)There may also be discharge from the nipple, and the nipple can appear flattened against the breast
Doctor :   (3.)In approximately half of patients with Paget disease of the nipple, a lump or mass in the breast can be felt during physical examination
Doctor :   4.Paget disease of the nipple can also be found only on the areola, where it may resemble eczema, a noncancerous itchy red rash
Doctor :   (5.)Although rare, Paget disease of the nipple can occur in both breasts And most of the times it is Unilateral.
Doctor :   May I suggest you a few steps to rule ot anyhting big ?
Doctor :   1. Biopsy of the flakes of the skin of the left breast would be the best step.
User :   OK that is good to know for future reference
Doctor :   2. A mammogram would be the second step if detected positive in biopsy.
Doctor :   3. But in 90% cases these turn out to be simple allergies or due to dryness of skin, but I would say better to rule out.
User :   I agree, better save than sorry
Doctor :   I feel it is best to go to a Mastlogist or Gynecologist specialised in Gy.Oncology.
Doctor :   Exactly right.
Doctor :   Thank you for consulting me, do you have anything else to ask ?
Doctor :   Take acre,
