User :   I am having sharp pains in my rectum and also this morning have notice sharp pains near my right shoulder blade. I have noticed also for about 2 months that all of my bowel movements are NOT solid. I am 32 and I do smoke and drink occasionally. Any idea what this is?

Doctor :   Can you describe the pain that you have in the rectum?
Doctor :   Do you notice this pain only while passing stools or other wise too?
User :   Well just general discomfort most of the time. and I have the occasional sharp pain a few times a day. Also, just have a general bloated feeling alot.
User :   No not while passing stools very often

Doctor :   Do you feel the urge to defecate very often?
User :   yes i suppose but can not always go
Doctor :   Ok.
User :   when i do it is never solid
Doctor :   Do you have problem of heart burns, burning pain in the upper part of the abdomen?
User :   no not really heartburn. the burning pain in the upper part of abdomen: yes, i have notice that but it doesnt really hurt

Doctor :   May I know your height/weight/occupation?
User :   5'6/150/ XXX
Doctor :   Since when have you noticed the shoulder blade pain?
Doctor :   Can you describe this pain?
User :   i woke up with it this morning. havent exercised or done any lifting in a week! sharp pain esp when i breathe in
User :   almost feels like what i would describe as a stabbing pain
Doctor :   Do you have cough/fever?
User :   no
Doctor :   Are you able to move your shoulder without any discomfort?
User :   havent taken temp but dont feel as though i have fever
Doctor :   Do you have discomfort while turning your neck?
User :   no. it pretty much hurts constantly. not unbearable pain but VERY uncomfortable. no not while turning neck
Doctor :   Ok.
Doctor :   Have you tried any pills/pain killers?
User :   yes aleve
Doctor :   Did it help you?
User :   it isnt really a muscle (pulled) pain. at least doesnt feel like that. no feels the same
Doctor :   Do you have diabetes/hypertension?
User :   no
Doctor :   Ok.
Doctor :   Let me explain it to you.
Doctor :   Your two problems appears unrelated to each other.
Doctor :   let me explain your first problems the rectal discomforts.
Doctor :   You have symptoms suggestive of Irritable bowel syndrome.
Doctor :   The rectal pain probably may be related to this problem.
Doctor :   Alternative cause for the pain may be probable fissures in the anal canal.
Doctor :   The exact cause however can be determined by a per rectal examination alone.
User :   would that rule out colon cancer?
Doctor :   Based on your history, it does not appear to be a gut cancer.
Doctor :   But a clinical examination alone can be able to confirm it.
Doctor :   I would suggest you to get an examination done by a specialist which would include a perrectal examination and colonoscopy to rule out other possible gut issues.
Doctor :   Have you followed what I am saying?
User :   yes
Doctor :   In the mean time you need dietary modification which would include regular food habits,
User :   ok
Doctor :   plenty of fibrous food,
Doctor :   restriction of caffeine and the spices,
Doctor :   plenty of water,
Doctor :   and equally important is good sleep and proper stress management.
Doctor :   This is regarding your first problem.
Doctor :   Have you followed what I suggested?
User :   i guess my question on that is that i dont eat spicy foods really. and i dont notice the heart burn or burning in upper abdomen much.
Doctor :   These are few dietary modifications which are helpful in not only the gastritis but the IBS too.
User :   ok
Doctor :   Most often it is the diet and the stressors which are the causative factors for IBS.
Doctor :   Hence concentrate on the diet changes , stress management, moderate physical exercise, meditation with regular sleep.
Doctor :   Have you followed?
User :   yes
Doctor :   You can type if you have any queries regarding your first issue, while I explain about your first problem.
User :   ok
Doctor :   Based on the history that you have provided, the probable cause for the shoulder pains is a muscular pain, probable related to the posture of your sleep.
Doctor :   It should settle down with analgesics, heat fomentation, good rest and restricted shoulder use.
Doctor :   Aleve is a good analgesics, you may continue with it/
User :   so that wouldnt indicated anything having to do w my lungs or anything then?
Doctor :   If it is not providing you the desired benefit, then alternative analgesics such as Ibuprofen 400mg may be used.
Doctor :   Since you do not have respiratory symptoms, your symptoms are not related the the lungs.
User :   ok thank you

User :   yes
