User :   Hello. I have a question about pain in the inguinal area.
Doctor :   hi
Doctor :   Dr AAAAAAA here
Doctor :   please proceed with your health query
User :   hi dr. AAAAAAA ..
User :   i saw my PCP yesterday about pain in my groin...
User :   she pressed to see if I had any pain...
User :   i didn't...
User :   but on the left side I did notice that it felt like she sort of ran into something...
User :   i was paying attention for pain and forgot to mention that...
User :   not smart, i know...
User :   i am very overweight... could it be that because there's a layer of fat there that it didn't hurt, just felt like she bumped into something?
Doctor :   please provide your age, height, weight, details of past medical illness and treatment if any
User :   24; 6'; 335; hypothyroid; had gallbladder removed; bile salt diarrhea; labral tears in both hips; torn rotator cuff
Doctor :   do you have any swelling or discomfort or pain coming on in your inguinal area when your cough, sneeze, strain or lift anything heavy?
User :   not necessarily the inguinal area when doing that stuff; just periodic little pains there even when sitting; but more so in the middle of my pelvic area when coughing, laughing, running over bumps in car
User :   i do have discomfort and swelling there, though
User :   i also just began taking cipro for burning while urinating
Doctor :   in that case it could be due to inguinal hernia, a careful examination and ultrasound scan will help in identifying the problem.
User :   so not likely a muscle pull/strain?
User :   Doctor?
Doctor :   if it was due to muscle strain or pull, the pain would be mostly constant
User :   oh, OK. And can I wait till the end of the year to pursue this since my medical benefits change? Or does this require immediate attention?
Doctor :   if the pain and symptoms are not interfering with your daily activities, you can wait
User :   it does hurt somewhat to exercise
Doctor :   but if these are coming on daily basis and interfering in daily life, then you should hurry up
Doctor :   and avoid actions and activities that aggravate or bring on the pain
User :   OK. Again, just to be clear, the pain is not constant, so it's unlikely that it's a muscle pull, and even though I didn't feel pain when she pressed, and I did feel like she bumped into something, it could still be inguinal hernia?
Doctor :   yes it could still be inguinal hernia, since you said that you have mild discomfort and swelling in the area.
Doctor :   there are special examination tests that can be done during examination to confirm it
User :   even if I'm overweight? might be more difficult to tell?
Doctor :   i would suggest you to go ahead with evaluation done by a general surgeon to confirm/rule out inguinal hernia.
User :   OK. Good idea.
Doctor :   no, being overweight will not interfere very much in hernia examination
User :   Also, for surgery, could I go ahead and get that done even with the weight?
Doctor :   as the special tests will reveal if it is present
Doctor :   yes, you can go ahead with that , you will require surgery with 'mesh' put in to prevent recurrence until you lose weight and come back to ideal weight range
User :   Thank you doctor.
Doctor :   have you followed what I have said?
User :   Yes, I think so. Just one more question. I'm trying to figure out how I got this... I've been using the leg press, lifting some heavy things for landscaping at home. But I'm wondering if I could have possibly hurt myself masturbating? Would that change the diagnosis?
Doctor :   no that would not change the diagnosis
User :   ok, thank you for your time, doctor
Doctor :   being overweight is one of precipitating factors combined with increase in abdominal pressure
Doctor :   due to your exercises
Doctor :   is there anything else I can help you with?
User :   yeah, that's what I was afraid of
User :   no, i think that's all for now. thanks again.
Doctor :   thanks for consulting me. bye for now.
User :   bye
