User :   I'm a 41 year old woman with numerous health & chronic pain issues. I had a DVT in my right leg in 1998 & have had to wear compression stockings since. We've been on vacation since July 1 & are finally arriving home in about 6 hrs. I contacted my doctor on because both ankles were very swollen & I was concerned w/my DVT history (I know to move every hour, I pump my feet, etc to keep circulation going, etc. & I take a 325mg aspirin daily). She said to take a mild diarrhetic (which I've been doing since ) & I ALWAYS drank a lot of water, limit salt, have been elevating my feet & icing as much as possible. THE ISSUE: Was leaving the hotel @ 9AM & got sharp pains under my left breast (starts midway & goes around my left side about 2-2.5 inches). It's now 1 PM & it hasn't stopped & feels better when I apply pressure. Does this sound like an embolism? It's Sunday, so I can't call my doctor until tomorrow & want to get home, but if it is serious, we will go to an ER ASAP. PLEASE HELP!
Doctor :   Hi
Doctor :   Let me read the query.
User :   Hi
User :   Ok, I'll be waiting
Doctor :   Can you describe the nature of the pain?
User :   It feels like a stabbing/pinching type pain.
Doctor :   Ok.
Doctor :   Do you feel the pain radiating to the limbs?
User :   No
Doctor :   Do you have any breathing issues?
User :   Just mid breast & left in a straight line about 2-2.5 inches
User :   No
Doctor :   Do you have nausea or vomiting, sweating?
User :   No, I feel fine except for that spot that started out of nowhere
Doctor :   Ok
Doctor :   Do you have diabetes/hypertension?
Doctor :   Have you had similar symptoms in the past?
User :   The swelling in my ankles/legs is MUCH better
Doctor :   Do you smoke or consume alcohol?
User :   No to all of your ?'s except for the pain part.
Doctor :   Ok.
Doctor :   I shall explain it to you.
User :   I've had it happen a few times in my life (literally a handful, but it didn't continue) & it has been years since the last time.
Doctor :   Your symptoms are probably a benign natured problem, most likely a muscular ache and not really an embolic or cardiac pain.
Doctor :   They are medically described as a non cardiac chest pains.
User :   I'm just concerned due to the past DVT history & the swelling issues this week.
Doctor :   However, in view of your past history, it would be beneficial to have it evaluated by a cardiologist.
Doctor :   I understand that it is sunday today,
Doctor :   Hence I shall suggest you these simple measures.
Doctor :   1. To visit the ER if the pain is severe and is disturbing you.
Doctor :   Else you can use simple analgesics such as tylenol along with prilosec OTC one tablet each stat.
Doctor :   3. You would need good rest.
User :   Oh, one thing I forgot--I had surgery on my right shoulder on 4.21.10 & I have an OT that comes to my home 3 times/wk. I always do my OT 2-3x a day, but didn't have much time while on vacation due to the swelling issue.
Doctor :   That is fine.
Doctor :   4. And lastly not to neglect these issues and to have a cardiac evaluation whenever feasible.
User :   Could it be a muscle thing since I use pulleys daily & those resistance bands to get the shoulder moving again?
Doctor :   It may have been caused by these exercises, but with your age and the past history, its would be good if you can get the cardiac examination done in the future.
Doctor :   Its just to rule out that these symptoms are not due to cardiac issues.
Doctor :   Have you followed what I have suggested?
Doctor :   Hi
Doctor :   Are you there?
User :   Yes.
Doctor :   Have you followed what I have suggested till now?
User :   Had to buy 15 more mins so I could read what u wrote
User :   Yes
Doctor :   Do you have any further queries?
User :   The pain hasn't increased, but is the same. I will call my dr tomorrow. How do I keep a copy of our chat so I can forward it to her & keep for my records?
Doctor :   I shall help you.
User :   Also, I take numerous meds for pain issues, osteopenia, FMS, degeneration of my joints & lots more. Could the hot weather cause/contribute to this? IF it is a heart issue, what would b common?
Doctor :   In order to get the chat print, you would have to go to the home page of our website after you have logged in.
User :   Is there a way to get it emailed to me?
Doctor :   And then click on the account and setting option.
Doctor :   You will have an option of view my chats.
User :   Will it be in my acct permanently? I'm 7+ hours away from home, so I can't print & I'd like to email to my dr today
Doctor :   You can take the print of the chat from here.
Doctor :   The chat which we had now can be viewed any time by you.
Doctor :   But I shall try sending this chat details emailed to your id if possible too.
Doctor :   Do you have any other query?
User :   Ok, then I should b good. Do I have mins left 4 u 2 answer the heart ? Above?
Doctor :   Yes.
Doctor :   Your symptoms are not suggestive of a major cardiac events.
Doctor :   But since most often minor cardiac problems as they are asymptomatic, I have suggested you the cardiac evaluation.
User :   I had an EKG prior to surgery 2.5 months ago + all was fine.
Doctor :   Its purely based on your history that you have provided to me.
Doctor :   You can take those previous EKG's too, to your doctor for the evaluation.
Doctor :   Do you have any other query?
User :   Ok, sounds great & thanks 4 your help. My prior DVT ruined my life (can't have kids, had for 2 MONTHS B4 the dr even listened & I saw him numerous times), so I'm sure u can understand my worry.
User :   Have a nice weekend.
Doctor :   Things will be fine.
Doctor :   Thanks for consulting me. Bye for now. Please refer your friends to our service.
