Doctor:Good afternoon Doctor
User:what are probable causes for this
Doctor:Before I can answer that question of yours I would like to know a little more about the condition
Doctor:By sore throat, do you mean pain in the throat?
User:not always, only change in voice
Doctor:What kind of change have you noticed in your voice?
Doctor:has it become horse?
Doctor:I mean Hoarse?
Doctor:okay, has the hoarseness increased over time?
Doctor:Do you have any recurrent bouts of cough?
Doctor:Have you noticed any other significant issue around your neck, like a swelling in the front of the neck or any small nodes which are hard?
Doctor:And if you don't mind, may know your age and gender please
Doctor:Are you there?
User:i am 36, femal
User:no other thing\
Doctor:Do you have cough?
User:thyroid profile is normal
Doctor:Have you consulted an ENT doctor for this problem?
Doctor:if yes what did the doctor say?
Doctor:Do you have any redness or inflammation in the back of the throat?
Doctor:How did this sore throat start? Did you have any bacterial or viral throat infection 2 months ago, and that is still continuing, or is it like you are getting the throat infection very often?
User:how could i see that
User:not infection
Doctor:It is difficult to see the back of the throat
Doctor:I wanted to know if you have tried seeing it, then did it look red?
Doctor:Just another question, Do you smoke? If yes, how many? And where do you live?
Doctor:Hello mam
Doctor:are you there?
Doctor:Are you there mam?
User:no, i dont smoke
User:i live in delhi
Doctor:I guess you are busy, I will try to be brief
Doctor:Okay mam
Doctor:The change in the voice could be due some common reasons like vocal cord overuse
User:can be
User:what is the treatment
Doctor:or it could also be due many other things
Doctor:The other worrying cause is Thyroid cancers
User:if thyroid is normal
Doctor:but since you said there is no swelling in the front of the neck
Doctor:The thyroid hormones can be normal till very late stages in some forms of thyroid cancers
Doctor:so that one needs to be further evaluated
Doctor:other reason for hoarseness of voice is chronic pharyngitis
Doctor:it could be bacterial or viral
Doctor:and some times the reason could be an infection around the trachea or esophagus (retropharangeal abscess)
Doctor:In my practice I would advise an ultrasound scan of the thyroid gland after a thorough examination of the gland physically to see if there are any small nodules in the thyroid
Doctor:If there is anything like that then I would suggest an FNAC (fine needle aspiration cytology), a small needle based biopsy
Doctor:Before any of these, an ENT specialist's review will be appropriate to rule out the simpler and benign conditions which I mentioned
Doctor:I am suggesting you to visit an ENT doctor because you have been having this issue for 2 months, if the duration was less I would have suggested some home remedies
Doctor:Though, for the time being doing salt water gargling will help you along with steam inhalation and giving rest to your voice for a few days
Doctor:Please do consult your ENT doctor and ask him/her to examine your thyroid along with looking at the back of your throat and vocal cords..
Doctor:Rule out all possibilities
Doctor:Hello mam
Doctor:are you there?
User:i have visited an ENT
User:upon 90 laryngscopy, there are nodules on the vocal cords
Doctor:That explains everything
Doctor:That is why I had asked if you had visited an ENT
Doctor:The problem is vocal nodules
Doctor:the treatment for this condition is Voice rest
User:that is actually not possible
Doctor:I guess it is quite possible, but takes a strong will mam
Doctor:You have to stop talking loud
Doctor:or maybe you should just take a 2 week holiday and go to some remote location with your family and get rest
User:can i speak in normal voice
Doctor:You can speak lower than normal voice, or speak very minimal,
Doctor:voice rest means, not talk as much as possible
Doctor:I would suggest you ask your ENT doctor to give you medical leave certificate and take a leave from the work
Doctor:if your work is what is making you talk in loud voice
Doctor:if work is not the cause, the you will have to find a way yourself to sort out the issues
Doctor:Hello mam
Doctor:are you there

User:kindly suggest
Doctor:may I know the reason why you are not able to do voice rest?
User:as i am a sales trainer, with a part in business development
Doctor:Okay, Why don't you use a load speaker and a microphone if you have to speak to many people?
Doctor:Please go through my earlier suggestions, I have suggested you to take a vacation for 2 weeks
User:it is not possible
Doctor:Okay, What according to you is possible to do mam?
Doctor:what are your limitations and what are your options?
