Doctor:Ok I gone through
Doctor:the query
User:I am having the same problem again
User:It has been off and on for the past 2 months
User:I used the anti-fungal cream "Kety"
User:for the past 2 days. But I have not seen any improvement this time.
User:Last time it had started to improve within 2 days
Doctor:Do you see rash, itch and redness?
User:no rash, no redness
User:the area looks very dry
User:now there is more intense itching on the upper skin of the penis as well
Doctor:Do you have itching else where?
Doctor:Do you see pimple or pus filled lesions?
User:no nowhere else
User:no pimples
User:no pus filled lesions
Doctor:Does it happen only when you change clothes in the evening, right?
Doctor:Do you take bath before you change ?
User:no I do not take bath in the evening
User:small amount of itching happens during the day time
User:but when I change my clothes and get into home clothes, the itching gets intense
Doctor:How are the clothes washed and how frequently
User:washing machine
Doctor:Detergent allergy is very common
Doctor:Did you think of that?
User:hmmm. no
User:but nobody in the household is having this
Doctor:Use skin safe detergent
Doctor:No you are allergic to that detergent
Doctor:I am not saying that detergent is not good, but it may not be suitable for you
User:ok. How can I be certain that may be the reason
User:any reasons why the anti fungal cream is not working
Doctor:Get the clothes washed with very hot water and dry them indoors, not exposing to dust
Doctor:It may not be fungal infection
Doctor:Change the detergent and use safer alternatives
Doctor:IF you read the labels over the detergent, they so hypoalleric or non allergic
User:the same detergent we have been using for last 6-7 years
Doctor:Next thing is, apply CALAMINE with ALOE VERA over the area
Doctor:and stop rest of all the things
Doctor:Do not use undergarments
Doctor:to reduce moisture
Doctor:I also advise you to take hot water bath before you change
Doctor:the next possible allergen is the dust you travel from office to home
User:sorry did not understand "do not use undergarments"
Doctor:Does this happen even on weekends
User:yes, but more on weekdays
Doctor:Do not use undergarments while at home
Doctor:Get these changes and use the medicines and let me know after a week
Doctor:Did you follow?
Doctor:Should I summarize
User:yeah. only question I had was, this allergy can develop after 6-7 years
User:since I did not have problem earlier
User:I will summarize
Doctor:Otherwise I see no reason which is consistent with the description you gave
User:wash clothes in hot water. and dry indoors
User:2) Use Calamine with Aloe Vera
User:3) Use a hypoalleric or non allergic detergent
User:only other thing I wanted to tell you is that I go to the gym in the evenings
User:and I use the same undergarment that I was wearing during the day
Doctor:Good point to note
Doctor:Working out leads to retention of moisture
User:and some times I have bath only in the morning
User:could that be the reason
User:rather than the detergent
Doctor:After every gym session, a tepid water bath is
Doctor:Do not use the same undergarment
Doctor:Yeah a possible reason
User:i should still use Calamine only correct
Doctor:Nothing else
User:ok fine
Doctor:If these measures do not help, the next thing is a consult with Dermatologist
User:ok fine
Doctor:for detailed evaluation
Doctor:Do you have any more queries?
User:ok fine
Doctor:Thank you
User:no thanks
User:have a nice day.
Doctor:Bye for now
