User:Hello Dr. XXXXXXX .. I have been having a cold for some days..
User:its been over a week, but it has not got better
Doctor:Can you describe what exactly you mean by cold - do you mean nasal congestion, running nose and blocked nose?
User:yes..all of them.. and a ear ache in the morning sometimes
User:but no fever.
Doctor:Are you taking any medicines?
Doctor:May I know your age?
User:only crocin..
Doctor:May I know your age?
Doctor:You mentioned there is no fever, did you mean that fever has disappeared or you did not develop fever at all during the last 7 days?
User:the first day i did feel feverish, but nothing more than that after that
Doctor:Do you suffer from recurring nasal congestion from the past?
User:not really
Doctor:Do you have history of diabetes, high blood pressure or any other known medical illness?
User:yes i have diabetes..and am allergic to walnuts
Doctor:Let me brief you.
Doctor:You seem to have a condition known as rhinnitis. Here inflammation of the nasal cavity leads to nasal congestion, runny nose and blockage.
Doctor:The usual cause is allergy and acute infection.
Doctor:Many a time, this rhinnitis can be accompanied/spread to involve sinus, throat as well as ears.
Doctor:Your middle ear also seems to be involved.
User:im sorry, its rhinitis,right? I mean, single n?
Doctor:Though the long duration of symptom can bother all doctors, absence of fever is a positive finding.
Doctor:I apologies, yes rhinitis with a single N.
Doctor:Now, crocin alone would not suffice.
Doctor:You will need other measures.
Doctor:What I suggest my patients with similar problems are:
User:actually i have been suggested the same thing by another friend who is a doctor
Doctor:1. Steam inhalation. This is extremely effective. You can use plain warm water to inhale or add contents of Karvol plus capsule to the water and inhale. It can be done thrice a day.
Doctor:2. Take antihistamines. Cetrizine 10mg tablets at night for 3-5 days.
Doctor:3. Nasal decongestant like otrivin nasal drops for not more than a week.
Doctor:2 drops instilled into each dose. You can start of twice or thrice a day and over the next couple of days reduce to once a day and stop by a weeks time.
Doctor:In addition to these three steps, I would encourage to chew chewing gums for a day or two.
Doctor:It will encourage draining contents spread to the ear and help you.
Doctor:Yes, the following measures will be able to take care of rhinitis.
User:do i need a prescription for cetrizine?
Doctor:If symptoms worsen in the next few days, you will need to see a doctor for closer examination.
Doctor:Cetrizine is available over the counter in some locations; but in India this is classified as a prescription based drug.
User:but your sales person here told me that you never advice prescription drugs?
Doctor:If steam inhalation doesn't relieve your symptoms, you can visit a local physician and get a prescription of this antihistamines.
Doctor:Let me clarify, rhinitis is best treated with over the counter as well as prescription based medication.
Doctor:The advice I suggested is a classical treatment for rhinitis.
