User :   Hi Doctor. I have a query regarding HCG levels during pregnancy
User :   My HCG test shows a level of 6000. I wanted to know in which week should that level be seen?
Doctor :   HCG levels of 6000IU/L occur around 6-8 weeks of pregnancy
User :   I was checking on the net and I read in various articles that this is at 4 or 5 weeks of pregnancy though.. is that possible?
Doctor :   On an average such levels are seen after completion of 6 weeks, during the 7th week.
User :   I had a scan 2 days back and at that time the sac could be seen but the fetal pole was not developed nor was there any heartbeat...
User :   my gynaec advised that it was probably because the fetus was around 5 weeks but she had said that the hcg would be around 2000...
Doctor :   The levels of the HCG depend on the growth and size of the fetus.
Doctor :   The levels are not very constants
Doctor :   Such levels can occur normally during 5-8 weeks broadly.
Doctor :   It is definitely high in the 4th week.
User :   but does a level of 6000 indicate that the fetus should have been more developed by now? is it a cause of concern that the fetal pole cant be seen even though level is 6000
Doctor :   But such values can occur any time between 5th week to the completion of 7 weeks and rarely in the 8th week.
User :   Are you there Doctor?
Doctor :   If fetal pole and the fetal heart rate are usually seen by 5weeks
Doctor :   and if fetal heart rate is not seen during this time, its probably a concern.
Doctor :   Do you have the scan reports with you?
User :   no
Doctor :   An ultrasound scan is necessary during early pregnancy at around 5 - 6 weeks to establish the presence of fetal heart rate.
Doctor :   Presence of fetal heart rate most often indicates presence of normal pregnancy.
User :   I have an irregular menstrual cycle and my LMP date was 2nd Dec. It sometimes happens once in 2 months so we dont really know when ovulation happened...
Doctor :   Ok
User :   so we are trying to correlate the hcg level of 6000 with the development of the fetus to determine the number of weeks and whether the development is normal...
Doctor :   Let me explain to you.
Doctor :   There are 2 possible conclusions which I can conclude based on your description,
Doctor :   1. The pregnancy is a blighted one, probably due to structural abnormalities of the ovum
Doctor :   2. The pregnancy is too early.
Doctor :   It is difficult to differentiate between these 2 conditions, unless we have a serial HCG levels or repeat Ultrasound scans.
User :   are there any other indications or symptoms of a blighted ovum?
Doctor :   Unfortunately there are no symptoms that can differentiate a blighted ovum and a normal pregnancy, this early.
Doctor :   Symptoms such as reduce height of the uterus, are usually seen later in the pregnancy.
Doctor :   The condition is mainly differentiated by looking at the HCG levels and by ultrasound scans.
Doctor :   My suggestion at this point to you is not to get disheartened now.
Doctor :   Get a repeat scan after a week to see the progress of the pregnancy.
User :   ok
Doctor :   If it is a blighted ovum, it is a natures way of preventing an abnormal fetus from developing.
Doctor :   Have you followed what I have suggested?
User :   Yes.. I have a scan scheduled for 9th.. next wednesday
User :   my doctor has given me duphastone tablets to support the pregnancy
User :   but i read somewhere tht this should not be taken during pregnancy.. is that correct?
Doctor :   The role of this medications varies.
Doctor :   The tablet contains synthetic progesterone, and are used in conditions where there is deficiency of HCG,
Doctor :   Reduced HCG levels can increase the chances of early abortion.
Doctor :   The medication is prescribed to prevent this.
User :   but isnt my level high enough to stop this
Doctor :   Since you are not aware of your gestational period and the scan isn't conclusive, the advantages of the pill in preventing early abortion is considered significant.
Doctor :   Hence I agree with your gynecologist in giving you this drug.
Doctor :   If scan reveals a normal pregnancy which correlates with the HCG levels then this medication be stopped.
User :   ok
Doctor :   Your next scan along with HCG levels will be conclusive in deciding the further course of treatment.

User :   Yes it is.. but I have had a biochemical pregnancy 2 years ago

User :   is it common to have some abdominal cramps during this time?
Doctor :   Minor abdominal cramps can be occur during early pregnancy.
Doctor :   A severe cramps needs evaluation.
User :   okay.. No I have been having mild cramps a few times
