User:Hi my sister
Doctor:How can I help you?
User:has a problem.
User:I am suspecting it to be thyroid
Doctor:Are you there?
User:symptoms are: from past 2 to3 yrs she gets cold(flu) suddenly for no reason and it there is a sticky water flowing continuosly
User:she is growing fat and losing hairs
User:she feels a tickely sensation in throat and nose
User:sneeze and feels tired
User:so is this thyroid or something else?
Doctor:May I know her age?
User:she is 32
Doctor:Has she got her thyroid hormones checked?
Doctor:Cold or nasal congestion has few reasons. Thyroid hormones has nothing to do with this.
Doctor:Recurrent nasal condition may be due to infection (flu or other bacterial)
Doctor:or due to allergies
Doctor:or due to pre-existing nasal problems such as deviated septum, sinus issues and so on.
Doctor:Similar hair issues and weight gain can have other causes.
Doctor:It's too prejudgemental to say she suffers from thyroid problems
Doctor:unless we have blood reports suggesting abnormal hormones.
User:but this is happening from around 3 to 5 years and now days she is getting cold more frequently
Doctor:I would therefore suggest you to get her thyroid function tested. Also see her ENT surgeon to understand if she has a nasal problem.
Doctor:As I said, she getting cold (nasal congestions) has nothing to do with thyroid problem.
Doctor:Low thyroid can lower body temperature.
Doctor:This is not the same symptoms as you describe.
User:we showed to an ent doctor and he has given few tablets..i am typing them below
Doctor:Nasal congestion and flu like symptoms are due to infection, allergy or pre-existing nasal problem
Doctor:which needs to be determined by ENT surgeon.
User:defort 6mg,odimont fx,allenil,and nasal spray
Doctor:All these drugs are given to treat allergic condition.
User:we feel she is having hearing loss as well
Doctor:If the cause is allergies, she needs to find her allergens and avoid it.
User:how do we find that?
Doctor:Ear and nose is linked by tube (eustachian tube). Some of nasal problems get transmitted to the ear through this connection.
Doctor:SO if you feel she has hearing problem
Doctor:report back to the ENT doctor
Doctor:or take a second ENT opinion
Doctor:and get her treated.
Doctor:You can ask her to keep a diary.
Doctor:Record all things that happen before she develops symptoms.
User:what are the symptoms of thyroid?
Doctor:If you find any common factor (agents) then you might be able to figure out the allergens.
User:she was treated for tonsils a\and undergone a operation in childhood
Doctor:Thyroid disorder is of two types - hyperthyroid (overactive state) and hypothyroid (underactive state).
Doctor:Hypothyroidism cause weight gain, dry skin, menstrual irregularities, hair problems, cold intolerance (low body temperature) and so on.
Doctor:Please note that cold intolerance is because body temperature is slightly low, so any exposure to cold causes shivering and patient prefer to stay in warm environment.
Doctor:This is a totally different symptoms from what you describe - nasal congestion and flu where patient has blocked nose or runny nose.
Doctor:Nasal congestion is due to allergy or infection
Doctor:and as I said earlier pre-existing sinus and nose problems can cause recurrent congestion.
Doctor:Have you followed?
User:yes but she feels cold and wears sweater
User:when she gets cold
Doctor:That's natural. Febrile episode secondary to infection
User:what is hypothyroid (underactive state).
Doctor:would make a person lose temperature.
Doctor:The cold and shivering sensation that occur during those symptoms are related to body's mechanism to loose temperature.
Doctor:Moreover symptoms of hypothyroidism is constant and remains so until we correct the hormone deficient.
Doctor:In simple terms, if she was hypothyroid cold and other symptoms should be constantly present
Doctor:and not once in while occurrence which improves without medication.
Doctor:I hope you have understood.
Doctor:I do not say she doesn't have low thyroid. I only say that nasal congestion is not related to thyroid.
User:ok can u tell me the tests we need to conduct to confirm if or not it s thyroid and what type it is?
Doctor:You can get her thyroid hormones checked to know if its causing her weight gain and hair fall issues.
Doctor:You can ask for thyroid function test.
User:The nameof test?
Doctor:This test includes three components in it.
Doctor:It is referred to as thyroid function test
Doctor:or you can ask for TSH,
Doctor:T3 and T4 level check.
User:why does this happen in first place?
Doctor:It is a blood test. Preferably get this done early in the morning.
Doctor:The exact cause is unknown. Some people it is due to autoimmune disease.
Doctor:Some people suffer because of hereditary/genetic component.
User:also is there any test for detecting the agents which cause allergy?
Doctor:Some other suffer because of low iodine intake.
Doctor:we also have a group of patient who develop this post pregnancy and post infection.
Doctor:But majority of people have this problem due to unknown reason.
Doctor:To know which allergy causes allergic symptoms is not an easy task; both for doctors and patients.
Doctor:Since there are thousands of allergens, it is a very cumbersome process to test all allergen.
Doctor:Therefore I suggest patients to keep dairy and figure out common exposing factors.
Doctor:It may be a diet, a plant or dust or food product or any other factor.
Doctor:If she is unable to figure the allergen
Doctor:she can visit a specialist who will perform
Doctor:skin patch test using the most common allergens
Doctor:the successful result of skin patch is unpredictable.
User:right there is nothing specific she eats to get happens all of sudden more during cold seasons and in early mornings
Doctor:So keeping the dairy is the best way to look out for allergens.
Doctor:Cold and dust are common allergens found in people.
Doctor:we can do very little to prevent them except to take precautions during those time.
Doctor:As many a time, we see bunch of allergen
Doctor:acting together
Doctor:encourage her to figure out through a dairy
Doctor:if there are any other allergen such as a plant, place, food or any cosmetic product
Doctor:that precipitates the symptoms.
User:ok one more thing...regarding migrane
Doctor:If any is found, avoiding those will reduce the problem.
Doctor:Proceed with your next question.
User:I and my sis get it say once a month or once in 2 to 3 months
User:so basically the eye's get blured and the view becomes patchy and dark
User:and then it starts paining in head
User:from one side to from left side of forehead to to right
User:it either happens in one eye or both and some time reaches upto back of head as well
User:it stays for one day complete and slowly subsides the next day
User:So why does this happen and what is the treatment
Doctor:There are various theories that are proposed to be the cause of migraine. Changes in neurotransmittor chemicals of brain and changes in vascular tissues of the brain
Doctor:are two such theories which are held responsible for migraine.
Doctor:Unfortunately a clear cause of why migraine happens is also not known.
Doctor:Since we do not know the cause, we aren
Doctor:*aren't able to figure the complete cure for migraine yet.
Doctor:We rely on preventing headaches and symptoms of migraine.
Doctor:In both your case, since migraine attacks are less frequent (once in a month/2-3 month)
Doctor:you don't need preventive medications.
Doctor:Take the available pain killer early when you feel you are going to suffer from migraine.
User:I had heard from a friend that his mother also had and she took some tablet for 3 to to 6 months every day and then it eraditcated it and from past 2 years she does not have migrane there any such tablet?
Doctor:You also need to avoid fasting, sleeping at irregular intervals, walking into sun and using a strong perfume.
Doctor:These thing can start migraine very often than not.
Doctor:The medications that your friend's mother took are preventive drugs.
Doctor:They don't eraticate migraine, they just prevent them from happening.
Doctor:Like I said earlier, we use preventive drugs when migraine attacks are very frequent.
Doctor:Fortunately in both your and your sister's case
Doctor:these attacks are not frequent to warrant those medications.
Doctor:You can visit a doctor for such medications, if you get frequent migraines, say once or more than once a week.
Doctor:Have you followed?
User:so this is when in one day u have many attacks or in one attack every day
Doctor:We suggest preventive drugs when migraine occurs once or more than once a week
User:yes can suggest such medice name..i will offcourse take only after consultation
Doctor:or if they occur for say more than a month and the headache is untolerable or uncontrollable with common pain killer.
User:one last thing so after my sis has food her stomach bulges a lot..and it happens every it because of gas or anything else?
Doctor: XXXXXXX let me remind you preventive drugs have some side effects. It is not worthy to try the medicine when migraine attacks are less frequent as in your or your sisters case.
User:ok..i wont take them just wanted name for reference
Doctor:But since you asked, we use antidepressant like amitriptylline or anticonvulsant like pregabaline
Doctor:as preventive medications.
Doctor:You can contact a doctor if migraine attacks become very frequent.
Doctor:you might consider trying these drugs if your less frequent migraines are not responding to common pain killers.
Doctor:Hope it is clear to you.
Doctor:Do you have any questions?
User:one last thing so after my sis has food her stomach bulges a lot..and it happens every it because of gas or anything else?
Doctor:It may be due to indigestion / hyperacidity associated condition or due to bowel problem.
Doctor:It may also be secondary to thyroid problem as you suspected.
Doctor:Without a clinical examination, it would be extremely hard to predict what the cause of abdominal bloating is.
Doctor:You can ask her to visit a doctor.
Doctor:Have you followed?
Doctor:Do you have any other query?
User:she has no problem with bowels..its just when she has food her stomach bulges..and it happens regularly
User:as u mean to say it is one of the symptoms of thyroid?
User:will get it tested
Doctor:I meant, the abdominal bloating may be due to hyperacidity associated condition, bowel/intestinal problem such as irritable bowel or due to systemic disease including hypothyroidism.
Doctor:Without clinical examination and testing, it is hard to predict the real cause.
Doctor:Getting her physically examined and tested will reveal the true cause.
Doctor:Hope it is clear.
Doctor:Do you have any more questions?
User:ok I well get the blood test for TSH
Doctor:You are welcome.
User:no questions for now
Doctor:Thanks for consulting me.
Doctor:Bye for now.
