User : i have type 2 diabete and am suppose to take 2 units of insulin if my sugar is above 200 and 4 if above 250 and use the sliding scale so i basically have no idea how that works
Doctor : Hi xxx,
Doctor : This is Dr yyy here
Doctor : let me read your query.
Doctor : ok
Doctor : May i know your age, and how long have u have had Diabetes ?
User : 33 and i found out i had diabetes three months ago
Doctor : what are the medicines that you are on?
Doctor : other than insulin?
Doctor : and how long have u been on insulin?
User : my sugar level stays above 200 pretty much daily and has been as high as 680 i was prescribed metaform??sp but it makes me sick
User : taking insulin for two months
User : daily for the last month
Doctor : metformin, is a good drug to stabilise the sugar levels
Doctor : So about insulin, what doubts specifically do u have ?
User : i am suppose to use the sliding scale but i have no idea what that means
Doctor : sliding scale, - is used to regularise the usage of insulin
Doctor : a high blood glucose means a higher dosage of insulin
User : all i was told was that if above 200 take 2 units and above 250 take 4
Doctor : if less than 120 - no insulin is needed
User : ive never been less than 190
Doctor : 150-200mg sugar - take 1 unit
Doctor : upto 250 - take 2 units
Doctor : greater than 250 - take 4 units of insulin
User : ok this afternoon i was at 300 i took 4 units and right now i am at 257 do i take 4 units again?
Doctor : yes u can take 4 units again
Doctor : 4 units, is not a big dose, and u need not worry
Doctor : this is just to replace the lack of insulin production in your body
User : ok i just dont under stand what or how the sliding scale is suppose to affect my intake i read on the internet something that basically said my intake should increase
User : something that contradicted what i was told and thought i was suppose to increase my dosage
Doctor : what do u mean by intake ?
User : dosage
User : of insulin
Doctor : yes, sliding scale, refers to dosage of insulin
Doctor : it rises as the blood glucose levels rise
Doctor : if your BGL is low, the required dose decreases
Doctor : if its high the dose needed increases
User : ok so how do i use it when taking my insulin lets say tommorow morning my level is 280 do i still just take 4 units
Doctor : yes 4 units
User : and if at lunch im at 330 just 4 units?
Doctor : if less than 199, take 1 unit
User : ok i understand now
Doctor : if its above 300 u need to take 6 units
User : ok
Doctor : for every 50 mg increase in sugar, u need 2 units of insulin
User : ok perfect
Doctor : i hope u have understood my advice
User : yes i understand now thank you
User : one more question
Doctor : you are welcome ,
Doctor : sure proceed
User : how hard is it on my body for my level to average around 200-220 daily for the last thirty days my level has averaed 248
Doctor : it depends ont he diet
Doctor : you eat
Doctor : Hi Are u there ?
User : yes i am back
Doctor : ok
Doctor : for the question you have asked ,
User : the hospital told me to eat a diet of high carbs which made no sense to me so what i have been doing is tring to just eat a healthy diet
Doctor : it depends, basically on, the diet you eat on a regular basis and regular exercise
User : i am not really sure what I am suppose to eat i test myself several times a day just to get a feel for what does what to my levels
Doctor : high carbs , increase the blood sugars , so u have to eat it moderately
Doctor : reduction in high saturated fats, like cheese, butter, etc is needed as these are harmful
User : ok can you suggest a website that i could reference for diet information
Doctor : using olive oil, etc is good as they aheve high qty of unsaturated fats, thats good for u
Doctor : give me a minute
Doctor : WWW.WWWW.WW Doctor : is a good place to start
Doctor : other than this, there are a lots of sites on the net, where u can gain additional info as well
User : ok well thanks i think that is it for now
Doctor : thanks for consulting me
