User :   i have had a minimal amount of a clearish sticky discharge from one duct, left breast only but not from the nipple itself but more underneath the nipple for over a year. I had a lumpectomy a few years ago, in which it was benign from the left breast. I have developed itching under left armpit over past couple of months, sometimes extending to upper outer of left breast. I generally have lumpy breasts anyway and now it kind of feels gritty in different spots.
Doctor :   Hi
Doctor :   let me read ur query
Doctor :   may i know ur age, weight, height , please ?
User :   i am 33, 56kg, 164cm
Doctor :   ok
Doctor :   thanks
Doctor :   is this discharge near the lumpectomy scar ?
Doctor :   are u there ?
User :   the discharge comes out of the nipple but not if you just sqeeze the nipple itself, the lump was below the nipple at about 6 o clock and was about 2cm diametre
Doctor :   so do u feel any swelling below the nipple ?
User :   its now really an empty space
Doctor :   ok
Doctor :   so there is some space that feels like a hollow there ?
User :   kind of, if you milk the areola it kind of clicks
Doctor :   this could be due to some collection of fluid that results as a result of the empty space there
Doctor :   this fluid could result due to percolation from the surrounding glands
User :   meaning
Doctor :   when a surgery results in a empty space, that space will be filled up either by tissue or body fluids
Doctor :   this body fluid could be inter cellular fluid
User :   i had a blood test for hormornal prolactin levels, but it that was not the case so maybe ur got it
Doctor :   the fluid that is between tissues as a lubricant
Doctor :   u mean to say the prolactin levels were normal ?
User :   Yes, there was nothing there! i had to have the suturing undun a week following the lumpectomy, and was really sick
User :   they had to let the wound granulate by packing it every day for bout 7 weeks
Doctor :   ok , so there was some sort of infection following the lumpectomy , and they allowed healing by secondary intention
Doctor :   ok
User :   until it healed, yep
Doctor :   This very well explains this
User :   the scar is the whole bottom half of the nipple
Doctor :   Do u have any pain or foul smell in the discharge that u have now ?
Doctor :   ok
User :   no pain, discharge doesnt smell
Doctor :   ok
User :   breasts really lumpy um feels gritty
Doctor :   do u feel that when you lift your arm upwards, there is dimpling in the breasts ?
User :   like grissle in meat kinda
Doctor :   i understand
User :   not really persay
Doctor :   this feeling results due to fat tissue in the breast , condensing to form small nodules within the subcutaneous layer of the breast skin
User :   right
Doctor :   this is sometimes due to the effect of chronic infection
Doctor :   and inflammation
User :   i m a type 1 diabetic?
Doctor :   ok
User :   but white blood cells been ok
Doctor :   that is fine
User :   so what do i do
Doctor :   the findings that you have described seem to be benign nature
Doctor :   this is just a subcutaneous changes in the skin of the breast
Doctor :   Some signs that might indicate cancer in the breast include a lump that is new or feels unusual to you. It is not like the fatty or glandular tissue
Doctor :   regarding the discharge, in the breast , the fluid can be drained by a suction using a syringe, after proper examination
User :   there isnt that much discharge though
Doctor :   and then the cavity can be closed by using certain tissue adhesives
User :   oh
Doctor :   In that case, it can be left alone
Doctor :   and no need for any interventions
User :   do i need to get any of the smooth palpable lumps checked or they ok
Doctor :   yes , you can get them checked the next time you go , just so that you dont have any doubts about them
Doctor :   some additional tips that can be helpful
Doctor :   include
Doctor :   Oil of evening primrose may help, either in the form of a gel capsule by mouth (300-400mg per day in divided doses) or as a cream to be applied to the skin
Doctor :   Applying warm pads to the breast.
Doctor :   Wearing a good support bra.
Doctor :   and vitamin B complex and vitamin E supplements
Doctor :   these supplements and measures can help improve the skin tesxture, by dissolving the abnormal fat globules
User :   ok sweet, thankyou for your time much appreciated Doc.
Doctor :   I hope i have answered your queires
User :   yes thankyou very much $ well spent Doc.
Doctor :   please leave your feedback at the end of the chat
Doctor :   thanks and take care
Doctor :   have a great day
User :   will do cheers
