User :   my brother have diabetes, he is taking combination of Glimepiride and metformin. still his sugar is 180 both fasting and PP. AGE - 40

Doctor :   hi
User :   sir, what should he do now
Doctor :   sugar control in diabetes happens with diet, regular exercise and medications
User :   he is doing all
Doctor :   if after following a good diabetic diet and doing regular excercises , medications are not controlling sugars,
User :   for last one year his sugar stands at this level
Doctor :   then alternate medications use should be dicussed with the treating doctor
Doctor :   and if stil sugar levels fail to come back to normal
User :   example
Doctor :   then insulin should be tried after treating doctor advises for it
User :   but after few years he may develop resistance for insulin, he is only 40 years
Doctor :   alternate medications include insulin modifiers, and other more effective medications than what he is already taking
Doctor :   you can discuss with an endocrinologist

User :   that why oi am discussing with for next line of treatment
User :   what is insulin modifiers
Doctor :   the sugar levels being consistently high suggest insulin resistance
Doctor :   that he could already be into
