User:Hello Good Evening
Doctor:Good evening
Doctor:How are you doing?
User:I had some issue which I think would be good to be discussed with a male doctor
User:Can any male doc. chat with me now or shall I ping later
Doctor:I can assure you that I am capable of handling your queries
User:Ok, i believe you
Doctor:Please proceed with your query
User:for last few days i am continuously getting irritation , pain & burning sensation below foreskin.
Doctor:Has it become excessively red?
User:Just to give you some back ground, I had possible genital herpes infection occured @ 1.6 years back. I never got sores and had done blood testing in two labs and one lab says no infection and other said on border level
Doctor:Herpes is known to recur
User:I went to skin specialist and he started treating me for Herpes, I am getting this issues time and again.
Doctor:Actually, the virus goes and lodges in one of the cluster of the neurons
Doctor:Whenever the immunity goes down, or in some other triggers
Doctor:It gets reactivated
User:OK, In the mean time I had hydroseal in both testis and pain in right testis travelling to lower back, went to UTI speceliast and he told me I have got falrasis
Doctor:filariasis, right?
User:Yes, Yesterday i went to the same doctor who is treating me for last 1.6 years and he told me that although there are no visible sytmptoms but it can be herpes. Then I went to a skin and VD specialist in Pune who is practicing for 33 years and seems to be good docotr. When he checked me he told me that he dont think it is herpes, it may be a fungal infetion
User:So i am confused, about the issue. Can you pl. guide.
User:yes what do you think I should do
Doctor:The fact that it is recurring, does point towards herpes
Doctor:But herpes commonly causes small ulcers in the genital region
Doctor:along with that, there is fever, muscle aches and painful urination
User:Infact i never had it which can be atleast seen by naked eyes. Few days back I got a small red sore on penis head but it disappered in 1 hour no pain, tingling or burning. This is first time it has happened.
Doctor:The only conclusive proof is by testing
User:I only gets redness, pain and burning sensation
User:what testing ?
Doctor:There are certain investigations like polymerase chain reaction, virus culture
Doctor:in the secretion
Doctor:Can I ask you a personal question, if you dont mind?
User:As i said earlier I did blood test for HSV at two different labs, one says not reactive and other says on borderline
Doctor:did you have unprotected sexual intercourse with someone who was possibly infected
User:no intercourse only a massage and handjob few months back
User:i was under influence of alchol
User:got HIV tri dot test done for 4 times in last 1 .3 years all negative
Doctor:Then its unlikely that you have herpes
User:are tri dot tests conclusive?
Doctor:This is a rapid antibody test which is quite conclusive
User:Ok i had it at an interval of @3 months each
User:The massage was body to body massage, can it give me herpes?
Doctor:Its unlikely
Doctor:The virus is present in the secretions
Doctor:That need to enter a damaged mucosa
User:So i am totally confused and facing this issue for last few months, done know what to do ?
Doctor:I think you can do the following
Doctor:Take the treatment for the fungal infection
Doctor:MAintain local hygeine
Doctor:And totally avoid unprotected exposure
Doctor:candida is a likely possibility in your case
Doctor:and candida also recurs
User:I have been trying all sort of local medicines it gives me relief for few days and again the problem reoccures
Doctor:Most over the counter things have an anti-fungal in them
User:I have used anti-bactrieal, anti-viral, anti fungal till day
Doctor:dont use any drugs without a prescription
User:My wife also went to gyenoglogist today and get full inspection, she dont have any infection
Doctor:Thats good
Doctor:It is possible that you have candidiasis
User:The first doc is recommending me HIV test again
Doctor:You would need both a topical and oral anti-fungal
Doctor:Did you get the tests done before that episode?
User:Yes when I went to Dubai for employement
User:@ 4 years back
Doctor:and when was this episode?
Doctor:Its extremely unlikely that you contracted HIV from such an exposure
Doctor:But if you still want to be reassured, get a test done
User:No Episode, it was routine pre employement test . I believe HIV is only transmitted by unprotected vaginal or anal intercourse/ blood tranfussion only
Doctor:No, when you got that massage
Doctor:thats what I am talking about
Doctor:Thats exactly what I am saying
Doctor:That it is extremely unlikely that you contracted HIV from that massage/handjob
Doctor:HIV testing is not required
Doctor:But if you still wanna be reassured, get a test done
Doctor:ELISA tests are one of the best screening tests available
User:I know I have never done any thing unsafe which can put me in this position
Doctor:I agree with you
Doctor:Did you ask your physician why did he recommend HIV tests?
User:He said just like that.. beause the redness is reccuring. Can I ask you one ?, what do you think about blowjob and HIV, I have been reading lot of contridictory information on net
Doctor:Although it is theoretically possible
Doctor:Considering the virus is present in the saliva
Doctor:but a lesion needs to be present on the penis
Doctor:and that lady needs to be HIV positive
User:Do u knw a case where the infection has occured due to blowjob
Doctor:cases have been known to occur
User:ok , just asked as had this query.
Doctor:The risk is very small
Doctor:but it does exist
User:so what do I do now, contunieue treatment with the doc who is experienced and had give me anit-fungal ointment to apply for 30 days.
Doctor:and maintain hygeine
Doctor:Keep it absolutely clean aand dry
Doctor:And take a nutritious diet
User:Ya i clean the area and below forskin every day with warm water and soap
Doctor:Have you been screened for diabetes?
User:Yes i had seen my sugar level @ 6 months back, normal
Doctor:then we're good
Doctor:take the anti-fungal
User:But if my wife dont have infection and I have not done any intercourse outside then how can i get fungal or bactrial infection
Doctor:Its not transmitted sexually
Doctor:It can happen to anyone at any time
Doctor:Its seen more in diabetics
Doctor:thats why I asked you
User:can it reoccur time and again as I get relief some time and then again the problem starts
Doctor:It recurs
Doctor:But Effective treatment does help considerably
Doctor:but local hygeine is the most important in the long run
User:What diet do you recommend to prevent this ?
Doctor:take a well balanced diet
Doctor:preferably XXXXXXX in proteins
User:Eg pl.
Doctor:like dal, rajma, chicken, fish, egg
Doctor:avoid stress
Doctor:exercise regularly
Doctor:all this helps build immunity
User:Any medicien you recommend to boost my immunity and get releif from this recurring issues
Doctor:its not required
User:Pl. dont mis understand me, It is not my habit to do thing outside, some mistakes made
Doctor:Its fine
Doctor:as long as you take adequate precautions the next time
Doctor:It can be XXXXXXX
User:no i am not doing it next time
Doctor:both for you and your wife
Doctor:dont worry about the fungal infection
Doctor:It'll be ok soon
User:Ok , Just a last ?.... what is the possibilly of HIV transmisison in protected blowjob?
Doctor:Very less
Doctor:Almost minimal
User:Thanks for your valuable advise and time
Doctor:If you are that worried, get yourself tested and get it over with
User:I would continue with the treatment
User:Bye and take care
Doctor:Thanks for consulting me
