User :   21-year-old boy was having palpitation, depression, anorexia, insomnia

User :   his pulse rate is always above 100 and he tells me that 2 work load since 1 month and he had lost about 5 kg weight. I have prescribed him propranol 10 mg bid and alprazolar 0.5 mg at bedtime. but he still having palpitation

User :   daily he only gets about 4 to 5 hours of sleep. ECG, echocardiogram, CBC, T3, T4, TSH tests were performed which all tunred out to be negative

Doctor :   any other symptoms that he is having apart from the once that you have mentioned like fever or cough or any past history of medical illness
Doctor :   Types of tachycardias, are Atrial fibrillation, Atrial flutter, Supraventricular tachycardia (SVT).
User :   he denies any fever, chills, chest pain, shortness breath
Doctor :   ventricular tachycardias are of different nature
Doctor :   Some tachycardias are triggered or made worse with exercise.
Doctor :   Stress test will be the next modality of investigations that would be needed in his case
Doctor :   Apart from Coronary angiogram
Doctor :   As for the treatment measures are concerned then what you have started him on as of now is fine and it will still need to be seen as to what are the causes for his tachycardia
User :   tell me about medications that decrease the heart rates
Doctor :   also try eliciting history of any Drugs that he might be on
Doctor :   Calcium channel blockers, potassium channel blockers and beta blockers
User :   but i have already prescribed propranolo 10 mg b.i.d
Doctor :   anticoagulants, are not used to treat the abnormal heart rhythm, but rather to reduce the risk that the abnormal rhythm triggering a blood clot that may lead to a stroke or heart attack.
Doctor :   Yes that is why i mentioned that you need to evaluate him further for the exact cause of tachycardia
Doctor :   It would be advisable to send him to a cardiologist
