User :   For the past two years I have had to put Goldbond or Cortizone on my perineum and raphe perinealis. Every day I need to apply the goldbond or cortizone 1-3 times a day on the area. Its a burning sensation every morning and when I am sitting for a long time it starts to hurt and burn and feel chaffed but when I put the goldbond on it feels fine and doesnt hurt anymore for a couple hours. Sometimes I only need to put it on once a day but whenever I go to the bathroom and wipe I need to reapply it otherwise it burns again. There has never been any rash or bumps or wounds so I dont think its a STD or rash. Its starting to get really annoying and I dont want to have to keep applying it everyday. It seems like whenever I sweat a lot it gets worse. Like I said there are no bumps, rashes or anything that would make me think it was a STD. Maybe its cause I sleep with alot of blankets at night but I dont know. Anyway you can maybe tell me what may be going on?

User :   I dont see any rashes or skin changes. it looks all normal

User :   yeah he said there was no rash or bumbs

User :   no injury or anything in that area

User :   yeah
User :   at school it just gets uncomfortable

User :   not like painful
User :   well yes and no with sweating

User :   once the sweat dries then it hurts
User :   really no bad smell except for the usual sweaty smell. and no itching really sometimes it itches but not all the time
Doctor :   ok. was a swab test done to look for any local infection causing you the symptoms?
User :   no there was not'
Doctor :   if not done, then I would recommend you to request your doctor for getting a swab test done by taking a swab from the local area where symptoms are the worst.
Doctor :   it will help in identifying if any infection is present
User :   does it sound like a STD?
User :   someone said it could be genital herpes. but for the 2 years not one sore or rash
Doctor :   that is responding to the steroid crean that you are using by reducing in inflammation
Doctor :   the symptom of burning and occasional itching could be related to infections caused by STD, if you are sexually active
User :   i think i need a high power steriod cream
Doctor :   getting a STYD screening test will help in idetifying if there is silent infection

Doctor :   yes it could be genital herpes without outbreak and causing mild symptoms. only by getting it tested, we can confirm/rule it out
User :   it seems to be the worst when i wake up. i sleep with 5 blankets i dont know why i do but i do and it hurts in the morning
User :   could it just be a fungal infection also that just wont go away without medicine being applied
User :   and how could genital herpes be on the perineum
Doctor :   fungal infection if suspected should be treated with antifungals
Doctor :   to make it go away completely
Doctor :   the swab test will help in identifying if it is indeed fungal infection
User :   could a fungal infection stay with u for two years if not handeled?
Doctor :   drink lots of water about 10 glasses per day, it will also help to flush out infection
Doctor :   yes, it can become chronic and last for as long as you do not treat it aggressively
User :   does it help that i eat a lot of fast food
Doctor :   if it is found to be a chronic fungal infection since sweating is involved
Doctor :   you might need oral plus topical/local application of antigfungals to treat it.
User :   i heard about drinking water that it could help
User :   ok i will go to the dermatologist and see what they say if the water doeswnt work for a couple weeks
Doctor :   try with home foods and simple foods with no additives. stick to having citrus fruits and juices
User :   ok. anything else you can suggest using over the couinter?
Doctor :   and lots of vegetable salads tossed with a tinge of olive oil, it might help
