Doctor :   Hi
Doctor :   this is Dr xxxx
Doctor :   how may i help you today
User :   My husband started vomiting a dark borwn/blackish substance on Friday morning. He only puked about 4 times. He felt lathargic for the remainder of the day and into Saturday. He complained of an upset stomach. Yesterday it began all over agin except the vomiting was extensive (8-10 times). I assumed as the vomiting conmtinued it would begin to get lighter in caller since all he was drinking was water, but it has remained black. He has a history of severe heartburn that he only treats with an over the counter medication. He recently had some mild anal bleeding as well that he attributed to a hemmeroid. His father died of colon cancer. Any thoughts?
Doctor :   let me read what u have typed
Doctor :   to start with, can u let me know ur husbands age, weight ?
Doctor :   also i would like to know if he has a history of smoking or alcohol intake
User :   He is 46 and 240 pounds at 6'4".
User :   Not a smoker. Light drinker.
Doctor :   ok
Doctor :   has he had such episodes prior to this?
Doctor :   is he having any medical condition other than heartburn?
User :   He often pukes when his heartburn gets the best of him, but not to this extent. He usually pukes once and then moves on with his life.
Doctor :   ok
User :   No. He is generally in good shape. Worksout 5-6 times per week.
Doctor :   so this is the first time that he has vomited a brown/ black vomitus ..
Doctor :   ok
Doctor :   this could be a result of chronic heartburn , also called as Gastro Esophageal Reflux disease
User :   I suppose it's been that color before, but we assumed the color was due to the contents of his stomach.
User :   At this point he should be puking nothing but bile, no?
Doctor :   No , this brown / black vomitus - could be due to a ulcer in the stomach or lower esophagus ( tube connecting the throat to the stomach)
User :   So, the color is blood?
Doctor :   due to which , some amount of bleeding occurs in the stomach and presents as black or brown vomit ( blood is alterd by the acid in the stomach)
Doctor :   yes, this black or brown vomitus could be blood
User :   Obviously, he needs to get to a doctor, but is this an emergency situation?
Doctor :   Unless he is taking any iron medications
User :   No iron supplements.
Doctor :   this condition needs attention and he has to be seen by his physician, and proper evaluation, including his stool examination, complete blood check (to rule out anemia), has to be done
Doctor :   if he is feeling very weak, and drowsy, and he is not bale to eat well OR if he is vomiting very high amounts of blood, then he should go to the ER and be seen immediately
